Associate Social Worker

Job Overview

The Associate Social Worker is responsible for the development, implementation and evaluation Behavioral Health counseling with individuals and groups. The Associate Social Worker reports to the Director of Behavioral Health.

SPECIFIC DUTIES:

  • Conducts comprehensive intake interviews including medical and psychosocial assessment and development of treatment plans for clients in assigned service areas
  • Provides counseling and case management services for individuals and families including clients with substance abuse and mental health problems
  • Monitors client needs and service provision and conducts follow-up as needed
  • Develops and maintains linkages with other health and social service agencies
  • Develops and facilitates support groups for men, women, teens, parents and/or school-aged children
  • Conducts and/or participates in assigned family unity meetings and workshops
  • Provides client advocacy activities including coordination of services for medical and dental care, emotional and practical support, public assistance, community services, and mental health services
  • Maintains client documentation in compliance with agency protocols
  • Participates in case conference activities to coordinate care for clients
  • Prepares written reports as assigned by supervisor
  • Assists with the preparation of reports to funded sources as needed
  • Performs other duties as assigned

MINIMUM REQUIREMENTS:

  • Master’s degree from an accredited school of social work
  • California licensure as an Associate Social Worker
  • Ability to obtain LCSW licensure within 24 months from date of hire is required. It is preferred to obtain LCSW licensure within the first 12 months of date of hire.
  • Experience with integrated behavioral health services within a medical home health facility.
  • Experience with integrated behavioral health services working with populations that present with mild to moderate behavioral health difficulties.
  • Experience with crisis intervention and substance use disorder techniques and interventions is preferred.
  • Public speaking ability.
  • Experience counseling individuals and families is preferred.
  • Valid California driver’s license, proof of auto insurance, and personal transportation
  • Preference for individuals with the ability to communicate in Spanish and English; verbal and written.
