- JOB SUMMARY
- Provides daily case management, advocacy, and support to consumers with mental health, substance abuse, and/or medical needs in a culturally sensitive and patient centered manner.
- Educational Skills Required (Degree(s), Certification(s), License, etc.)
- Bachelors degree in social work or counseling field required
- Valid NJ Driver s license required
- Work Experience Required
- 1 year of case management experience preferred
- Clinical experience working with behavioral health population, crisis intervention, and/or case management in a hospital setting preferred
- Commitment to working with clients with compassion and empathy for under-served populations
- Knowledge and understanding of urban issues, hospital procedures, community resources
- Strong verbal and written communication and organizational skills to effectively work and communicate with clients, service providers, hospital staff and TRAC team
- Computer literacy
- Punctual and reliable
- Able to work in stressful situations and problem solve, including verbal de-escalation of consumers
- Bilingual Spanish preferred but not required
