Behavioral Health Case Manager
In order to prevent a direct threat to the safety and health of our patients and staff, effective August 1, 2021, new hires are required to have had at least the first dose of a COVID vaccine prior to their start, with the exception of those who have documented medical or religious exemptions. New hires who are not yet vaccinated can arrange to have the vaccine on site.
This position is available starting December 1, 2021.
POSITION SUMMARY:
The Behavioral Health Case Manager provides crisis consultation/assessment on an urgent basis, support and linkage services, advocacy, education, brief interventions and counseling, and information in order to ensure adequacy of health care, increase access of services, enhance physical and psychological well-being and facilitate patient self-care.
Behavioral Health Case Management patients are patients who are referred by BNHC medical providers and who are at high risk due to medical diagnoses; who are frequent users of the hospital emergency rooms; and who have emotional, social or environmental concerns that impede their ability to function optimally.
Principle Duties and Responsibilities
1. Respond to patient referrals by assessing and evaluating all relevant information and developing a care plan. The assessment and care plan shall be communicated both orally and in written form with the patient. The document is maintained in the patient’s electronic medical record.
2. Collaborate with members of the interdisciplinary primary care team to evaluate patients’ medical, behavioral health, and social service needs.
3. Provide information, referral, and advocacy to patients in terms of mental health services, entitlement benefits, day care, housing, legal aid, prenatal care, and other social services in the community. Create and maintain liaisons with appropriate agencies.
4. Provide short-term solution-focused interventions and crises intervention to all patients and their families, taking into account their unique cultural backgrounds. Psychosocial education around such issues as development through the life cycle, social/emotional impact of chronic illness will be provided to patients and their families as appropriate.
5. Participate as an integral member of interdisciplinary health care team for the provision of services to patients and families. As a member of the primary care team, this position is integrated into a variety of clinic activities such as team meetings, protocol and procedure development, the provision of and receipt of staff in-service training and program development.
6. Provide technical assistance and training for other members of the primary care team and work with them to integrate psychosocial care into a variety of service components, such as health education activities, nutrition services and patient adherence.
7. Participate in outreach activities that involve the promotion of the Health Center. Demonstrates characteristics of accountability and responsibility.
8. Participate in community and agency outreach activities that identify, recruit, and/or educate our patient population.
9. Is pleasant, courteous, and considerate of patients and co-workers.
10. Maintain patient confidentiality at all times.
11. Able to work independently as well as working with the Care Team.
12. Attend all required and pertinent meetings.
13. Attend supervision as scheduled.
14. Maintain patient documentation in a professional and timely manner.
15. Maintain caseload appropriate to clients referred, and achieves a target of 25 patient encounters a week.
16. Perform projects as assigned by supervisor.

Qualifications:

Bachelor’s Degree in Social Work or related field from an accredited program. Two years experience working in a health care facility providing social services, demonstrated success as part of a multi-disciplinary team and commitment to diversity may be considered in lieu of BSW/BA.

 

Minimal Knowledge:

Knowledge of client advocacy and local service agencies. Knowledge and skills to conduct assessment and treatment plan, inclusive of appropriate goals and interventions. Basic computer skills.

Working Conditions

A busy community health center located in a downtown business district. Occasional long hours may be required.

Physical Requirements

Physical demands requiring lifting of small pieces of equipment and/or boxes not to exceed 20 pounds. Mostly sedentary, with interoffice walking required. Visual acuity sufficient for frequent reading. Hearing acuity sufficient for holding conversations with or without audio devices.
