Job Overview
Position: Behavioral Health Facilitator
Location: Centerville – Will require travel to other locations.
Summary: Assist in coordinating communication between psychiatric providers ensuring consistent patient care and coordination of services.
Experience: Qualified applicants must be well organized and computer literate. Education: High school diploma or GED. Preference will be given to candidates with CNA, CMA, or LPN license. Prior completion of Medication Manager class is preferred.
All employees are required to be fully vaccinated for COVID-19 prior to employment or must request an exemption no less than 10 days prior to start date.
