Inova Kellar Fairfax is currently seeking a Licensed Professional to join their PHP team as an Intake Coordintor.

Completes assessment and treatment plan development for adolescent individuals diagnosed with mental health and or substance use disorders. May be assigned to faciliate group counseling, crisis intervention and behavior management to achieve improvement of treatment goals and discharge dispositions. Adheres to Inova’s Culture of Safety Adheres to Inova’s Culture of Safety. Completes documentation of assessments, treatment plans, therapy sessions, case manager discharge summaries and formal written reports as needed, in a timely manner. Participates in maintaining a Professional Culture in Inova Behavioral Health Participates in maintaining a Professional Culture in Inova Behavioral Health. Participates in treatment team meetings and group supervision, as scheduled. Participates in activities integral to the maintenance and continued development of the Inova Behavioral Health Adult Ambulatory Services program. Adheres to Inova’s Culture of Safety. Provide a range of therapeutic services, psycho-educational programming and behavioral interventions to assist the client in achieving progress towards the goals and objectives of the individualized service plan. Reports to Manager. Provides case management Provides case management and coordination of treatment services for adult patients seeking services and education to their families, as needed. This may include coordinating with referring professionals, treatment team, QI, UR and others to ensure appropriate care. Conducts intake sessions to assess patient and family needs. This is a Full-Time role, M-F, 7:30am-4:00pm and 2 evenings.

Job Responsibilities

  • Completes assessment of patient needs and goals for treatment to create appropriate treatment plan.
  • Develops integrated summary to provide a history of patient treatment episodes and successes.
  • Develops subsequent treatment reviews/updates.
  • Completes intake to assess placement of patient into appropriate level of care.
  • Provides family counseling sessions/meetings as needed.
  • Assesses patient for stability to be discharged from program. Assist patient in setting up aftercare plan.
  • Assesses patient for at risk behavior at time of discharge and completes discharge summary within required timeline.

  • Ensures the provision of a physically and emotionally safe therapeutic environment.
  • Demonstrates knowledge of licensing regulations and accrediting standards.
  • Implements effectively and consistently the principles of Inova Kellar Center’s behavioral intervention program. Adheres to Inova’s Culture of Safety.

  • Completes assessment and treatment plan development for individuals diagnosed with mental health and substance use disorders.
  • Appropriately gathers and assesses information regarding the affect a client’s primary diagnosis and symptoms have on their mental and physical well being.
  • Analyzes and interprets relevant data to determine client diagnosis and treatment recommendations to promote successful management of symptoms and progress toward goals.
  • Collaborates with the client to formulate mutually agreed upon treatment goals and objectives with the client utilizing all available resources.
  • Completes periodic treatment plan reviews to assess progress made toward goals and objectives.
  • Completes documentation of assessments, treatment plans, therapy sessions, case manager discharge summaries and formal written reports as needed, in a timely manner.
  • Complies with documentation requirements, as evaluated by Supervisor. Written reports are completed within the required timeframe.
  • Participates in maintaining a Professional Culture in Inova Behavioral Health Serves actively on a committee as evidenced by committee attendance records and meeting notes
  • Participates in all staff and team meetings
  • Maintains licensure as required by job standards.
  • Demonstrates evidence of a minimum of 20 continuing education units (CEU) of credit yearly. Meets annual state license requirement for continuing eduction units CEU’s.
  • Participates in professional educational and training opportunities to further develop professional competence and improve program effectiveness.
  • Participates in development of curriculum and patient teaching materials, as evaluated by completion of assignments or projects.
  • Serves as a mentor to Program Therapist I staff members and interns.
  • Meets annual state license requirement for continuing education units CEU’s.

  • Helps patient to problem-solve by giving specific assignments or meeting one-to-one with patient, as needed, evaluated by observation and by peer review of patient record documentation.
  • Identifies and contains crisis situations by setting limits, and identifying and using appropriate resources, as evidenced by lack of negative outcomes.
  • Is prepared for each scheduled lecture and ready on time, as evaluated by observation, patient feedback, and chart review.
  • Teach patients to practice effective life coping skills, promotes experiential problem solving and reinforces learning and change through role modeling and counseling, as evidenced by direct observation and measured by patients’ ability to maintain an improved level of functioning and/or abstinence from substance use.

  • This may include coordinating with referring professionals, treatment team, QI, UR and others to ensure appropriate care.

  • Displays knowledge of adequate referrals in the field and patient area or residence and is able to provide prompt and appropriate referrals to service/program providers, including outside resources if necessary.
  • Provides case management.Participates appropriately in the treatment planning team collaborating with other professionals, as evidenced by direct observation.
  • Coordinates care planning with external professionals and completes required certification of care by required deadlines, as evaluated by peer review of patient records if indicated.

  • Develops appropriate discharge/transition planning and ensures client and family understand recommendations.
  • Provides documentation and performs administrative functions appropriate to clinical role
  • Develops an integrated & discharge summary for assigned patients which should accurately summarize patient data from all available sources, as evaluated by ongoing review of patient records.
  • Documents patients’ identified problems, negotiates a treatment plan for each with the patient establishing realistic goals and objectives, all within established policy guidelines.
  • Evaluated by peer review of patient records. Writes a clinical progress note for each treatment encounter.
  • Charts significant material as evaluated by review of patient records.
  • Participates in on-call rotation, when applicable.

Minimum Requirements

  • Master’s degree or PhD in mental health, social sciences, or behavioral health related field.
  • 2 years experience in addiction treatment/mental health treatment.
  • Professionally licensed by the Commonwealth of Virginia: Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), Licensed Clinical Psychologist (LCP), or Licensed Substance Abuse Profressional LSAP.
  • BLS obtained in 3 months and CPI obtained in 6 months of start date.
