CARE COORDINATOR

GRADE: 11 – Non-exempt REPORTS TO: Director of Care Coordination

POSITION REQUIREMENTS: The Care Coordinator must possess a minimum of a bachelor’s degree in human services, social work, or is a registered nurse. A bachelor’s degree in social work is preferred. It is preferable that the care coordinator have two years of satisfactory experience in the human services field working with the elderly. Must have computer knowledge and experience.

SUMMARY: Will assess client needs and coordinate services among all providers. The care coordinator will also develop resources to meet the older adult’s needs. Prepares reports and other materials as required. Maintain caseload and documentation on client files; performs data entry. Routine travel in service area is required. Significant public contact is required and position requires some independent decision-making and action.

DUTIES AND RESPONSIBILITIES:

1. Assesses client needs through the use of the Uniform Assessment Instrument (UAI), both short and long assessments.

2. Coordinates services for the elderly among human services agencies, including departments of social services, health, mental health, etc.

3. Determines client eligibility for participation in appropriate program, i.e., case management, information and assistance.

4. Develops a plan of care for each client.

5. Monitors progress of client’s services and needs.

6. Develops new resources to assist in provision of services, i.e., churches, civic groups, private sector, etc.

7. Maintains accurate records.

8. Input progress notes into case management drive and all units of service into Peer Place.

9. Accurately completes reports in a timely manner.

10. Maintain strict confidentiality concerning all information pertaining to clients and program operations. Full compliance with Health Insurance Portability and Accountability Act (HIPAA) rules is required.

11. Attends training as required.

12. Performs other agency-related duties as assigned.

KNOWLEDGE, SKILLS AND ABILITIES:

1. Knowledge of aging and the impact of disabilities and illnesses on aging.

2. Knowledge of conducting client assessments (including psycho-social, health, and functional factors) and their uses in care planning.

3. Knowledge of Consumers’ rights, local human and health service delivery systems, including support services and public benefits eligibility requirements.

4. Knowledge of the principles of human behavior and interpersonal relationships.

5. Effective oral (including active listening), written and interpersonal communication principles and techniques.

6. Knowledge of general principles of record documentation.

7. Knowledge of service planning process and the major components of a service plan.

8. Skills in interviewing techniques.

9. Skills in negotiating with consumers and service providers.

10. Skills in observing, recording, and reporting behavior.

11. Skills in identifying and documenting a consumer’s needs for resources, services and other assistance.

12. Skills in identifying services within the established services system to meet the consumer’s needs.

13. Coordinating the provision of services by diverse public and private providers.

14. Ability to analyze and plan for the service needs of elderly persons.

15. Ability to demonstrate a positive regard for consumers and their families.

16. Ability to be persistent and remain objective.

17. Ability to work as a team member, maintaining effective inter and intra agency working relationships.

18. Ability to communicate effectively, verbally and in writing.

19. Ability to develop a rapport and to communicate with different types of persons from diverse cultural backgrounds.

20. Must demonstrate and maintain excellent work ethics.
