The Care Coordination team at Nice Healthcare has an exciting opportunity for an experienced Care Coordinator! In this role, you’ll work collaboratively with the clinical team to create the best healthcare for our patients. We are looking for a motivated, resourceful, and organized individual to provide amazing services through patient care coordination and customer support.

What you’ll do at Nice:

Triage Coordination

  • Utilize the Nice app, EMR, and chat platforms to communicate with patients and staff
  • Respond to incoming messages from patients and triage communication between patients and clinical staff in a timely manner
  • Understand, prioritize, and triage urgent questions from providers
  • Send electronic medical forms to patients when requested by a member of the clinical team
  • Electronically fax patient referrals to specialty clinics
  • Answer the main phone line and triage calls appropriately
  • Monitor and triage all incoming emails and faxes
  • Track all incoming patient lab and imaging results
  • Ensure all lab and imaging results are successfully received and alert the appropriate provider
  • Assist patients and providers by troubleshooting connection issues
  • Provide support and break coverage for other members of the Care Coordination team
  • Ensure every communication and interaction between Nice and its patients is a positive and professional experience

Scheduling Coordination

  • Schedule home visits
  • Review visits for the day and plot a route for each provider based on patient’s needs
  • Electronically fax patient referrals to specialty clinics
  • Manage communication with providers via phone and/or Nice app messages
  • Check in with providers throughout the day to make sure they are moving through their routes efficiently and they have all the supplies/medications needed for each visit
  • Follow up on lab/imaging orders each day for patients

What you’ve done before:

  • Extensive experience with technology including, but not limited to web-based communication, virtual mapping platforms, virtual teleconferencing software, online password managers, software-based authenticators, MS Office and Google Suite
  • Ability to efficiently transition between multiple digital platforms
  • Experience troubleshooting technical issues, specifically within an iOS operating system
  • Strong attention to detail, prioritization, organization and self discipline
  • Extremely effective communication with team members using various web-based interoffice communication platforms
  • Ability to work efficiently in a virtual team setting while working independently from home
  • Minimum 1 year high-volume scheduling experience

Position Specifics:

Job Type: Full Time

Hours:

  • Monday-Friday: 7:30-4:30pm CDT (applicants). 8:30-5:30pm PDT (applicants)
  • Every fourth weekend: 8am-12 pm CDT or 8:30-12:30 pm CDT

Location:

  • Fully remote
  • A dedicated work space, free from interruptions is required

Job requirements:

  • Strong Internet signal with a secure wifi
