Job Description

The MBH Access Department is responsible for coordinating patient and client services in order to provide timely and quality access to Mental and Behavioral Health Services at Children’s Wisconsin. In conjunction with the patient’s provider, the Care Navigator is responsible for organizing and coordinating support services and treatment outside of Children’s Wisconsin for the patients and their families. Care Navigators provide community resources and make referrals, check in with families regarding at home therapeutic interventions the therapist prescribed, assist with navigating school services such as the IEP process, complete safety check-ins with families between therapy sessions or while a therapist is out of the office, attend team meetings and support the family while in higher level of care services, advocate for the patient/family when they feel their voice is not being heard, assist with guardianships, schedule follow-up services, navigate financial/insurance challenges, and remove barriers to receiving services. The Care Navigators are additionally available by phone to families in a crisis when they are unable to quickly speak with their provider. The Care Navigator is able to support the family through the crisis and connect with their MBH care team as needed.

Education: Bachelor’s degree (or equivalent knowledge) in social work, psychology, public health, healthcare administration or human services required.
Knowledge / Skills / Abilities :Exhibits guiding behaviors that reflect Children’s values and support our mission and vision.
Ability to work professionally with medical professionals to ensure successful follow up and coordination of medical management.
Demonstrates skills in creative problem-solving and outreach.
Ability to navigate, coordinate multiple services, prioritize and multi-task to meet the needs of patients, parents, and clinic staff.
Excellent communication and interpersonal skills necessary to gather and share information with providers, patients, families, and team members in a courteous, professional and confidential manner. Ability to maintain positive communication skills during stressful situations.
Committed to work toward improving one’s own cultural competence, i.e. valuing diversity, recognizing personal limitations in one’s own skills and expertise, and having the desire to learn. Accepting of individual differences.
Ability to work effectively independently and in a team setting in a fast paced environment.
Prior experience working in a Windows environment. Working knowledge of Microsoft Word and Excel.

Experience: Experience working in community programs with families of diverse ethnic, cultural and socioeconomic background preferred.
Epic experience preferred.
Knowledge / Skills / Abilities
Knowledge and experience in mental health preferred.
Knowledge of mental health insurance preferred.
Customer service experience preferred.

Additional Posting Description

This position is located at our Summit office in West Allis. Hours are Monday – Friday 8:30am-5pm.
