The Case Manager will report to the Clinical Supervisor and in collaboration with other members of the clinical team, is responsible for providing the necessary case management and support for our clients. The Case Manager will provide a range of services, including case management, assessment, service planning, referrals, crisis intervention and guidance in using available resources to assist families in meeting service goals.

Responsibilities:

  • Serve as a liaison for the clinical team, the client, the family, and all collateral service providers
  • Produce and maintain treatment materials and binders
  • Document all actions taken in connection with a case in a client’s medical record
  • Meet regularly with treatment team and caregivers to ensure proper implementation of the treatment plan, address specific target areas, review overall progress toward goals, and discuss changes to the treatment plan if needed
  • Ensure compliance, accuracy and quality of documentation and clinical records
  • Ensure compliance with Federal, State and agency guidelines related to administrative and clinical policies
  • Ensure clients receive quality and professional services
  • Respond to parent/caregiver/family concerns and determine appropriate resolutions
  • Participate in Clinical Team Meeting and other meetings as assigned

Required Experience:

  • Bachelor’s Degree with a major in social work, counseling, psychology or related human services field
  • Two years minimum experience working with children, youth and families in a home or community setting preferred
  • Experience working with children diagnosed with behavioral-emotional disorders or children with Autism Spectrum Disorder
  • Valid driver’s license with reliable and consistent access to a vehicle
