SIGN ON BONUS OF $900 & HAZARD PAY

A New Leaf’s licensed and accredited 24-hour youth shelter for children (ages 12-17) that offer specialized case management for the purpose of family reunification. Meals, education and warm place to sleep are provided along with activities to keep the children’s spirits up and deliver round-the-clock care.

Our dedicated staff provide a safe and stable environment in which to foster the optimal growth and development of each child. Supporting objectives include improved physical/Mental health, improved education/vocation skill development and timely family reunification and/or release planning.

We are searching for a full time Spanish/English Bilingual Case Manager to be a part of the already amazing and dedicated team.

Pay: $42K-45K DOE

Schedule:
1. Sunday-Thursday 9am-5pm
2. Tuesday-Saturday 9am-5pm
Location: Mesa, AZ

Primary Responsibilities:

  • Directly responsible to supervisor for administrative and clinical issues regarding admission, discharge and utilization of management issues, while being in conformance with accepted clinical practices and the agency’s program definition and policies (as relevant to the program/client needs.)
  • Develop and maintain collaborative relationships with clients, their family members, and significant support systems.
  • Develop Individual Service Plans
  • Screen for human trafficking concerns
  • Meet frequently with clients in-person and family members via telephone; provide up-to-date information on case status and new developments regarding progress and discharge planning.
  • Coordinate and assist with completion of all necessary documentation as designated by ORR and agency standards.
  • Document (verbal/written) and submit significant and routine paperwork such as progress notes, statistical reports, provision of services, daily activities, upcoming events, special concerns, varied incident reports, etc., as required by the program and submit to appropriate personnel.
  • Update and complete staffing notes for multi-disciplinary clinical and case management team reviews and ensuring that the treatment plan is carried out.
  • Co-develop Safety/Care Plans for clients, as necessary. This plan may be simple or complex, depending on the risk factors presented by the client and family.

Qualifications:

  • Bachelor’s degree in Behavioral Health, Human Services, or Social Services, required.
  • Demonstrate strong communication skills both verbally and in writing.
  • Child welfare and/or case management experience strongly preferred.
