Job Overview

The Case Manager works under the direction of the Adult Services Manager to support case management services for CommuniCare Health Centers’ CREO clients. Support includes, but is not limited to, facilitating communication with clients, primary care team staff, community organizations, treatment programs and other CommuniCare departments and outside agencies; clerical duties; and processing and maintaining databases. CommuniCare Health Centers is a Federally Qualified Health Center providing comprehensive patient-centered primary care services including medical, dental, substance abuse treatment, behavioral health and outreach services to the low income, multi-ethnic, uninsured and underinsured in the communities we serve.

Monday through Friday, 40 hours per week. 8:00 am to 5:00 pm. Evening hours and flexible work hours may be required. Travel to clinic sites in Davis, Woodland, and W. Sacramento required.

RESPONSIBILITIES
:

The following reflects management’s definition of essential functions for this job but does not restrict the tasks that may be assigned. Management may assign or reassign duties and responsibilities to this job at any time with reasonable accommodation.

  • Assist Behavioral Health Clinicians in working with patients with mild to moderate mental health issues by providing supportive case management, monitoring, and linkages to services.
  • Conduct screenings to determine eligibility for services
  • Collaborate with other CommuniCare programs to assist in the appropriate transfer of care and follow-up services.
  • Assist with coordination of care with entire CREO Team.
  • Responsible for tracking systems and case management, including scheduling follow-up appointments, scheduling appointments, assisting with referrals, discharge planning, and referrals to community resources.
  • Obtain and review charts or other data related to mental health treatment for follow-up appointment scheduling.
  • Provide logistical support for individual counseling appointments.
  • Provide reminder calls and letters to clients.
  • Assist with general health education and educational materials and distribution to clients.
  • Database entry into registries and tracking systems.
  • Administer or assist with other projects as needed.
  • Consult, collaborate and maintain positive working relationships with outside agencies to assist in client care
  • Provide presentations on services.

Education: Completion of M.A. training program preferred or Bachelors Degree in a related field.

Experience: Minimum of two years of medical/administrative experience in a social service, health related organization or medical office/clinic. Demonstrated knowledge of local health care delivery system. Experience working in a busy environment, often under pressure, without compromising accuracy, attention to detail, and courtesy to clients, staff and other agency staff. Demonstrated commitment to high quality health care for low income populations.

Skills and abilities: Bilingual Spanish/English required

  • Effective communication skills and ability to encourage patient understanding of prevention practices and early diagnostic services.
  • Computer/word processing skills required.
  • Experience with data entry into multiple database system (i.e. Access).
  • Must have excellent phone skills.
  • Strong organization and task management skills required.
  • Demonstrated ability to assume responsibility and to work independently as well as in a team.
  • Ability to work in a busy environment.
  • Knowledge of medical terminology strongly preferred.
  • Ability to demonstrate cultural sensitivity and polite communication with patients and staff members.
  • Current CPR certificate required.

Ability to operate office equipment, including computer equipment and phones. Hearing required. Ability to see colors and to bend and move arms in sufficient range of motion to complete patient interactions and train staff. Regular travel to all agency sites required. Must be able to provide proof of vehicle insurance and valid CA driver’s license.
