Job Overview

With the award of a new grant, Arizona Youth Partnership is launching a new program will develop a statewide system to deliver services for pregnant and parenting families in need. The focus of this project will encourage healthy pregnancy and childbirth, support childbirth as an alternative to abortion, promote family formation, aid successful parenting, and increase families economic self-sufficiency for families throughout Arizona. This statewide program will provide direct services, support services, social services, case management, and referrals to pregnant persons and parents (including adoptive parents) of children under the age of 2.

We are currently looking to hire for the position of Case Manager/Health Educator for Cochise County. This position will report to our main office in Marana, but will work remotely. The duties and responsibilities of this position include:

  • Develop a health education/case management plan with participants
  • Provide education and career guidance including referrals to education and jobs programs
  • Enroll prenatal participants into Pregnancy and Childbirth classes, parenting participants into Parenting classes
  • Facilitate Pregnancy, Childbirth, and Parenting classes; provide make up classes as needed
  • Track participant attendance/engagement in database; collect and report on all data as required by funding sources
  • Conduct ongoing case management sessions focusing on individual education and job/career needs
  • Maintain case plans and continue to meet on-going needs of program participants
  • Participate in ongoing outreach and recruitment of program participants throughout the community being served to include healthcare organizations (Federally Qualified Health Centers (FQHC), OB/Gyn providers, Pediatric Providers, Mental Health Providers, Pregnancy Care Centers), and other organizations such as community centers, adoption agencies, college/university campuses, high schools, jobs programs, faith-based, Department of Child Safety, Department of Economic Security (SNAP), local health departments, and WIC agencies.
  • Receive referrals for services from outreach efforts, media campaigns and from the 24/7 help line partner agency
  • Administer pregnancy tests and provide proof of pregnancy as needed and providing medically accurate information in response to client questions and pregnancy test
  • Enter referral and participant information in SOR database from help line, faxes, phone calls and walk-ins
  • Collect all necessary consent and confidentiality forms
  • Complete initial needs assessment including the Self-sufficiency Matrix, the Adverse Childhood Experiences (ACES) survey, and a Perinatal/Postpartum mood disorder assessment (EPDS)
  • Provide referrals as needed for prenatal care, pediatric care, mental healthcare, adoption information to community partners
  • Collaborate with pregnancy care centers and FQHCs to ensure participants are receiving services in their community
  • Ensures compliance with all agency policy, procedures and standards.

QUALIFICATIONS:

  • Excellent planning, organization and time management skills
  • Bachelors degree in related field or equivalent experience.
  • Knowledge of the local community and experience in community engagement
  • Direct service experience and experience working with, and linking families to clinical services.
  • Experience engaging providing outreach education to local and state stakeholders.
  • Excellent written and oral communication skills.
  • Bilingual English/Spanish – preferred

REQUIREMENTS:

  • Arizona Youth Partnership has a mandatory COVID-19 vaccine policy. You must either receive at least the first dose or submit an accommodation request prior to starting work. AZYP will consider all requested accommodations as required by law.
  • Ability to pass a background check
  • Participation in orientation and training as required by AZYP
  • Arizona Drivers License and reliable transportation
  • Auto Insurance of $100,000 and $300,000

  • CPR/First Aid Certification
