*PRIMARY RESPONSIBILITIES*
The primary responsibilities of the Pathways RRH Case Manager include working with Pathways 2 Rapid Re-Housing participants, assisting them in their housing search, providing case management services and monitoring the ONE System and coordinated entry referrals. This position can expect a caseload of 12 to 15 transitional aged youth and will be supported by the Program Manager and other Pathways Case Managers.

*Essential Job Functions: *

  • Client Outreach, Intake Assessments, Housing Application assistance, follow up during Intake Phase through Housing Move-In Date and up to two years of housing assistance.
  • Monthly home visits and regular weekly check-ins with clients who are housed or engaged in housing search.
  • Document Collection and Administrative Proficiency. Experience with client database and data collection.
  • Attend client meetings as needed including Roommate Meet and Greets, Housing Orientation, Unit Viewings, Lease Signing and Move-In Support.
  • Maintain communication with landlords, property managers, community partners, and assist with mediations, as necessary.
  • Attend RRH, ONE System and CES training and meetings as necessary to ensure accuracy of information and data tracking.
  • Communicate with Pathways team around case management and client needs.

*Other Duties & Responsibilities: *

  • Provide case management, mental health services, life skills development and housing services to youth in stages of changed based model.
  • Develop and maintain good clinical relationships with clients.
  • Collaborate with established community partners; ability to maintain positive working relationships.
  • Provide case management and independent living skills education to clients in the office, the field, and their home.
  • Crisis intervention and de-escalation techniques.
  • Practice life skills training for youth through modeling, active counseling and motivational interviewing and facilitation of groups in accordance with Core Curriculum Models.
  • Promote the wellness of the community through a harm reeducation approach.
  • Engage successfully with a diverse client population and demonstrate a commitment to Diversity, Equity, and Inclusion in all aspects of service delivery and programming.
  • Provide an intensive array of recovery-oriented services and supports, including housing and basic needs assistance, physical health care, benefits assistance, enrollment in Larkin Street’s education and employment services, and integrated mental health services.
  • Maintain accurate detailed electronic observation and progress records and support youth while adhering to confidentiality and reporting requirements.
  • Case Managers must be comfortable with clinical presentations, consultation and supervision and assertively participate in collaborative service planning.
  • Seek team support and supervision, when appropriate. Work with minimal to moderate supervision.
  • Participate in professional development activities to enhance skill levels.

*Physical & Language Requirements: *

  • Maintain professional standards of performance, demeanor, and appearance at all times.
  • Bi-Lingual Spanish is a plus however not required.
  • Perform tasks and responsibilities and complete in a timely manner, complying with agency policies and standards. Conforming to scheduling requirements of this job description and program.
  • Maintain a creative, team building, cooperative approach to job performance with a constructive, problem-solving orientation to challenges.

  • Requires ability to lift, transfer, push/pull, maneuver, and reposition 25 lbs.
  • Ability to reach, bend and walk.
  • Finger manipulation.
  • Ability to speak and write English clearly and accurately.

*Qualifications: *

  • Experience working with youth is highly desired, especially at-risk, homeless, or runaway youth.
  • Master’s Degree in Counseling, Psychology or Social Work preferred but not required or 3-4 years of experience working with high-risk youth in a shelter and/or transitional or supportive housing program(s).
  • Experience and knowledge of housing subsidies, property management, and tenant/lease relations strongly preferred.
  • Willingness to work from a strength-based, harm reduction, client centered model with an emphasis on evidence-based practices
  • Willingness to work with aggressive, difficult to engage and / or personality disordered clients
  • Ability to set clear boundaries with clients, other staff, and volunteers.
  • Ability to identify, assess and intervene effectively with mental health symptoms and behaviors
  • Ability to notice symptoms of substance use and abuse working knowledge of recovery and treatment philosophies.
  • Exceptional organizational and detailed orientated skills with reliability and consistency in work performance.
  • Excellent written and verbal skills, including public speaking
  • Ability to work with minimal to moderate supervision.
  • Ability to multi-task with the capability to learn quickly and integrate efficiently.
  • Ability to maintain accurate and timely documentation of clinical work
  • Must be a team player as well as have the ability to work independently.
  • Proficient in Microsoft Office (Word, Excel, and Outlook). Prior experience with client case management database collection.
  • Ability to provide HIV education and prevention
  • Working knowledge of issues facing homeless and LGBTQ youth.
  • Must be a self-starter with excellent follow-through skills.
