Job Overview

We are looking for a Case Manager to oversee files and case management young adults (16-24) in the Workforce Innovation and Opportunity Act (WIOA) Youth Program. The successful candidate will be accountable for all fourteen (14) program elements in the cycle of the program. Case manager duties include program orientation, assessing participant needs, program intake, service planning, implementing, monitoring and evaluating actions required to meet the participants’ service needs, maximize participant satisfaction and promote cost effectiveness. A certified case manager is a plus with or without Tier One Certification.

RESPONSIBILITIES

  • Manage a caseload of up to 30 participants and do what is necessary to foster success
  • Identify and respond to participants’ barriers by completing needs assessment and creating a Individual Development Plan (IDP)
  • Facilitate multiple care aspects (case coordination, information sharing, etc.) to the appropriate internal and external resources
  • Record and track participants’ daily attendance
  • Document changes and enter data in a variety of data systems that track program implementation
  • Report participants’ success stories for grant compliance and performance metrics
  • Complete accurately all necessary forms and produce statistical reports
  • Promote quality and cost-effective interventions and outcomes
  • Assess and address motivational and psychosocial issues
  • Adhere to professional standards as outlined by protocols, rules and regulations

SKILLS

  • Proven working experience in case management, including human service, social service, job coaching, medical, mental health care manager or a related job
  • Excellent knowledge of case management principles, workforce development, and alternative education options
  • Previous experience with psychological aspects of care
  • Effective communication skills
  • Excellent organizational and time management skills
  • Familiarity with professional and technical emerging knowledge
  • Problem solving skills and ability to multi-task
  • Compassionate with teamwork skills
  • Previous experience working with opportunity youth
  • Proficient in Microsoft Office products

EDUCATION

  • Bachelor’s degree in related field preferred
  • Associate’s degree in related field required

SPECIAL REQUIREMENTS

  • Must possess a valid FL Driver’s License
  • Must be insurable by the Authority’s fleet insurance carrier
  • Must be bondable

TRAVEL

  • Some travel may be required

Job Types: Full-time, Contract

Pay: $36,000.00 – $37,080.00 per year

Schedule:

  • 8 hour shift
  • Day shift
  • Monday to Friday

  • Daytona Beach, FL 32114: Reliably commute or planning to relocate before starting work (Preferred)

Experience:

  • Case management: 3 years (Preferred)
