SUMMARY OF POSITION: *
As our CCS Mental Health Coordinator, you will provide person centered recovery based care while implementing individualized goals and fulfilling our mission to provide a safe and healthy environment while envisioning, creating and committing to strength based quality services. A variety of hours is required due to needs of clients.

EDUCATION/EXPERIENCE:

  • Bachelor’s Degree in Human Services or Education. Experience preferred working with adult or youth with behavioral and emotional challenges strongly preferred.
  • Master’s Degree and coursework in areas related to mental health services, including clinical psychology, psychology, school or educational psychology, rehabilitation psychology, counseling and guidance, counseling psychology or social work.

KNOWLEDGE, SKILLS, AND ABILITIES/ ESSENTIAL FUNCTIONS:

  • Implement and participate in goal-orientated activities with the individuals. Including Wellness Management, Physical Health Monitoring, Individual Skill Development, building Social/Natural Supports, Psychoeducation, Socialization, Community Activities, Assistance/Guidance with transportation and Employment Related Skill Training.
  • Teach individuals various skills (social, life, employment, daily living) one-on-one or in a group setting.
  • Facilitate individuals engaging in appropriate physical/emotional activities.
  • Monitor the location of individuals when in services as deemed necessary to ensure safety.
  • Transport and accompany individuals to appointments, community activities and other locations as requested in the service plan.
  • Be knowledgeable in and able to implement and document properly all individuals’ services provided daily in required clinical documentation format which may include various computer systems.
  • Exercise good judgment, decision making with regard for safety. Communicate with Supervisor as necessary with situations that arise and assure proper procedures are followed.
  • Work closely and communicate with other members of the individual’s treatment team. This may include implementing therapeutic approaches discussed in therapy sessions, working closely with school teachers and communicating what occurs in a school setting, and implement skills and techniques as per treatment team recommendations.
  • Assist and/or supervise individuals with keeping activity and/or work space clean.
  • Provide in-home supports to specific families and provide assistance and coaching to parents, families and/or guardians.
  • Maintain a professional relationship with individual’s families, guardian and individuals in their environment.
  • Utilize training to de-escalate tense situations.
  • Meet all necessary training requirements in the timeframes necessary and attend all required supervisions as necessary to meet State requirements.
  • Service needs for individuals may vary frequently due to service needs/request of the individual’s family and/or funders. This means there is a potential of less than 24-hour notice of schedule changes.

MEETINGS:

  • Attend county meetings as well as required weekly staff meetings to meet statutory requirements.
  • Attend team meetings for individuals if requested.
  • Attend regularly scheduled program meetings and supervision requirements.

TRAINING RESPONSIBILITIES:

  • Attend mandatory in-service training and apply what is learned to the service needs of individuals.
  • Complete trainings required by state regulations within specified timelines.
  • Complete annual continuing education requirements, and apply what is learned to the service needs of the individuals.
  • Be knowledgeable in and observe all ODC practices, policies and procedures.

COMMUNICATION AND REPORTING RESPONSIBILITIES:

  • Implement and document properly all individuals’ services provided daily.
  • By law, this position requires employees to be mandated reporters. If ever there is reasonable cause to believe any individual has been abused or neglected, management must be notified immediately and a formal report is to be filed within 24 hours.
  • Employees are required to notify management during the duration of their employment if they have any outside relationships, either professionally or personally, with any clients and/or family members.
  • Monitor the health of individuals, document and report all changes of condition to manager and other parties as needed.
  • Report necessary incidents or concerns to the manager immediately or follow the proper chain of command.
  • The ability to communicate orally and in writing in the English language is required. Ability to write clinical documentation properly on a daily basis in required format on all individuals’ services is required. Ability to read and comprehend simple instructions and short correspondence are required. Ability to accurately document through written communication is necessary. Ability to verbally communicate in one-on-one situations and small-group settings is required.

OTHER SKILLS AND ABILITIES:

  • Employee must be able to properly work a cellular phone.
  • Employees are required to have general computer knowledge and be familiar with and able to work with Microsoft programs such as Word, Excel, and Outlook.
  • Knowledge of and ability to operate common durable medical equipment used in field such as glucose meters etc. may be required depending upon individual needs.

PHYSICAL DEMANDS:

  • While performing the duties of this job, the employee is regularly required to stand, walk, run, reach with hands and arms, talk and hear. Additional physical requirements to perform job duties are to push, pull, squat, bend, stoop, twist and kneel. Capable of lifting up to 50 lbs of inanimate objects and up to 35 lbs when manually assisting clients. Specific situations may require additional lifting capabilities.

WORK ENVIRONMENT:

  • Employees must be able to manage high stress environments. Good judgment and the ability to manage crisis situations are required. This position involves working directly with individuals who may present physical or verbal aggressions toward self, staff or others.
  • This position performs tasks that may involve exposure to blood, body fluids, or tissues.
  • Work locations during regular service hours include but are not limited to community settings, school settings and individuals’ home environment.
  • While performing the duties of this job, the employee is regularly in a client’s home, community, office, or setting with varying degrees of background noise. Lighting and ventilation are typical of a residential and office setting.
