Job Overview
We are currently hiring Certified Case Managers, this is a contracted position, all case managers are paid via 1099, this is a fee for service position. Not salaried base.
JOB SUMMARY: Responsible for providing Mental Health Targeted Case Management services to clients, under the supervision of the Case Manager Supervisor.
JOB RESPONSIBILITIES:
- Provides mental health targeted case management services as recommended.
- Utilizes experience and knowledge to assess needs of the clients and their families in order to link clients and their families with appropriate resources (such as housing, legal, academic, cash assistance, clothing, SSI, etc.)
- Completes and submits necessary documentation within deadlines for each client in accordance with local, state and federal guidelines and agency policies.
- Maintains ongoing communication with legal guardians.
- Act as a client advocate at all times.
- Maintains ongoing communication with assigned therapists/counselors and outside agencies involved with the client.
JOB REQUIREMENTS:
Education & Experience: Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education or a related human services (a related human services field is one in which major course work includes the study of human behavior and development) and have a minimum of one year of full time or equivalent experience working with children with serious emotional disturbances (S.E.D.)
OR
Bachelor’s degree from an accredited university or college and three years full time or equivalent experience working with children with serious emotional disturbances.
Have knowledge of available resources in the service area for children with serious emotional disturbances (S.E.D).
Certification: Current possession of or ability to complete the AHCA-approved mental health targeted case management training within three months of hire.
