Job Overview
Lakewood Health System is seeking a motivated Full-time* Clinic and Specialty Care Social Worker.*
This position will assist the clinic team with the planning, coordination and implementation of Social Services for our Family Practice and Specialty patients as deemed appropriate. This position is responsible for providing anticipated psychosocial and alignment of community resources to better care for our patients. This individual will assist with overall collaboration of patient care utilizing internal and external community resources and will work closely with the clinic team, providers and specific specialty lines of care within Lakewood Health System as well as collaborate with external, community resources.
This is a full-time position.
1. Bachelor’s Degree in Social Work
2. State license as a professional Social Worker or eligible to be licensed
3. Minimum of 2 years relevant experience
