Full Time Mon-Friday Must have the ability to work alternate shifts if necessary.

The Clinical Care Manager – (CCM) is responsible for the day to day coordination and oversight of all aspects of the Nursing Department in accordance with current Federal and State regulations as well as local regulations as assigned by the Director of Nursing. The Clinical Care Manager plans and provides managerial and/or clinical support to patients and staff within the Nursing department for health care – long term care –short term rehabilitation.

Knowledge, Skills, Abilities

  • Must be a graduate from an accredited program of professional nursing education.
  • Current license as a Registered Nurse in New York State.
  • Must be BLS/ACLS certified.
  • Two years of management experience is preferred…
  • Knowledge of application of current nursing techniques and procedures.
  • Knowledge of New York State Nurse Practice Act, New York State Department of Health Skilled Nursing Facility requirements.
  • Comprehensive knowledge of nursing service policies and procedures.
  • Must possess excellent communication and customer service skills. High-energy, articulate and intelligent person whose values, integrity, vision and enthusiasm that will inspire others.
  • Ability to work with a team, demonstrates a positive attitude and is a good role model for all nursing staff.
  • Must possess excellent computer skills and the ability to work with an electronic medical record
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess leadership and supervisory ability.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  • Ability to interact effectively with staff at all levels.
