  • Job Summary
    • Responsible for providing clinical case management to the patient population served by the KIDS Mobile Medical Clinic (KMC) by facilitating access to community resources and referrals and providing professional advocacy skills to both the patient and family. Provides intensive community/home-based support to high-risk families. The Child Advocacy/Mental Health Coordinator works to develop the Interdisciplinary Mental Health/Outreach Team that provides an array of developmental services to the KMC patient population and ultimately coordinates the providers and services available. Coordinates all developmental services for families of children birth through twenty-one years of age receiving primary healthcare from the Pediatric Clinic. Responsible for coordinating direct services with community based partner organizations related to child advocacy, mental health and social services. Responsible for direct provision of on-going mental health counseling where appropriate.

    • Education/Training
      • BSW is required. CONSIDERATION WILL BE GIVEN TO AN APPROPRIATE COMBINATION OF EDUCATION/TRAINING AND EXPERIENCE.
    • Experience
      • 1 year of related experience.
    • License/Certification/Registration
      • Current DC license as License Graduate Social Worker (LGSW) required.
    • Knowledge, Skills & Abilities
      • Knowledge of available resources and ability to act as liaison with other agencies. Must be sensitive to the needs of the community and the patients the Mobile Clinic serves. Skill in establishing and maintaining effective working relationships with patients, medical staff, and the public. Ability to react calmly and effectively in emergency situations. Able to effectively work in a team setting.
  • Primary Duties and Responsibilities
    • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Complies with governmental and accreditation regulations.
    • Oversees and provides clinical case management to families served by Mobile Clinic, including counseling, referrals, follow-up and advocacy. Provides intensive home/community-based support to high-risk families with needs inclusive of assessment, counseling, abuse and neglect intervention, emergency psychiatric care, and substance abuse treatment. Incumbent may work with a psychologist or LICSW in the home/community environment using therapeutic models to address intense and acute family issues and may provide direct on-going mental health counseling, where appropriate.
    • Coordinates service of mental health/outreach team for general pediatric sites and adolescent site. Coordinate psychiatric assessment services, child and family therapy services, psychology, speech and language, physical and occupational therapy. Work closely with all disciplines and follow up on recommendations for services and treatment. Work with Medical Director and program administrator to design evaluation tools and tracking systems to measure outcomes of the Mobile Clinic Child Development team. Develop relationships with other organizations where appropriate in relation to the mission of KMC and in conjunction with the Medical Director and Program Administrator.
    • Identifies, researches and develops community resources for patient population. Resources should address complex social needs inclusive of, but not limited to early intervention programs, Head Start programs, housing resources, mental health services, substance abuse treatment programs, legal advocacy, employment and training programs and mentoring programs. Make referrals to early intervention, childcare and Head Start programs, public schools, mental health services and other public and private sector agencies where necessary. Other resources determined as necessary.
    • Coordinates and participates in outreach efforts to promote services and to act as a liaison to other providers and community groups (schools government, churches, daycare centers, other agencies, etc.) where appropriate to improve service delivery for patients and families.
    • Attends individual family service plan (IFSP) or individual education program (IEP) meetings to assist families in planning and accessing community-based services. Works with attorneys and other professionals to advocate for the rights and benefits of the families the clinic serves. Attends community based/social service meetings that reflect our patient population.
    • In conjunction with the Medical Director and Program Administrator incumbent will participate, where appropriate, in program and grant development in relation to advocacy, mental health and programming appropriate to the mission of KMC and Georgetown University Hospital/MedStar Health.
    • Assists medical staff in teaching residents and medical students rotating with the community pediatrics program regarding social services and advocacy.
    • Participates in meetings and on committees and represents the department and hospital in community outreach efforts. Participates in multi-disciplinary quality and service improvement teams.
    • Performs other duties as assigned.
