Performs individual and group counseling and case management of assigned behavioral health patients to ensure coordination of services in accordance with established treatment plan


  • Provides individual and group substance abuse counseling
  • Serves as primary contact/clinical liaison to patients, patient’s families and community to ensure delivery of effective and appropriate treatment in accordance with established program
  • Provides community outreach and networks with other agencies and community resources
  • Reviews charts of assigned patients to determine appropriate referral services based on recommended treatment plan
  • Facilitates, schedules and/or participates in clinical team meetings to monitor patient progress against goals; updates progress notes and patient chart of all patient contacts within established timeframe
  • Collaborates with patient, family members and other involved parties to assess barriers to attainment of goals and/or resources
  • Responds to emergency and/or crisis situations; contacts appropriate individuals and/or agencies to ensure appropriate intervention and actions to optimize patient safety
  • Works closely and cooperatively with Tribal programs acting as point of contact for communication/coordination of care
  • Manages and oversees assigned case load to ensure delivery of services identified for continuity of care
  • Develops and implements transition, discharge and aftercare plans of assigned patients
  • Performs Behavioral Health assessment of patient, develops treatment/service plan to meet determined care needs; revises care plan as appropriate
  • Investigates and follows up on patient complaints to ensure timely response and/or appropriate resolution to concerns/issues
  • Identifies appropriate community resources and performs follow up as appropriate
  • Conducts patient visits to monitor progress and determine suitability of environment for successful treatment outcomes
  • Works closely and cooperatively with peers to review and/or revise standards, policy and procedure manuals based on findings and/or recommendations
  • Participates in departmental orientation, on the job training and quality assurance programs/initiatives
  • Participates in a variety of department and hospital educational programs to maintain current skill and competency levels; identifies and discusses performance or training needs with Supervisor


  • Master’s degree in Social Work or closely related field; equivalent combination of education and experience
  • Current State of Arizona license from the Arizona Board of Behavioral Health Examiners i.e. (LPC, LAC, LCSW, LMSW, LISAC, LASAC, LMFT, LAMFT)
  • Counselor hired with an associate license (LAC, LMSW, LASAC, LAMFT) shall obtain an independent license within 1 year of hire date.
  • CPR certificate within 30 days of hire
  • Must have at least 3 years previous experience; Native American population preferred
