Job Overview
The counselor position is responsible for providing therapeutic care to child, adolescent, adult, and geriatric patients during their acute inpatient hospitalization stay. ESSENTIAL FUNCTIONS:
- Completion of psychosocial assessments
- Participation in multi-disciplinary rounds, treatment team, and departmental meetings
- Care coordination with the patient’s community providers
- Facilitating individual/family/systems meetings
- Treatment planning/updates
- Leading clinical process groups
- Crisis intervention and debriefing with patients
- Managing the discharge and continued care planning process
- Maintaining documentation of all service provision
