Job Overview

REACH is a crisis prevention, intervention and stabilization system to support individuals with intellectual and/or developmental disabilities, as well as a mental health condition or challenging behavior that is negatively affecting their quality of life. REACH provides consultation, prevention, development of crisis education & prevention plans, community based mobile support and crisis therapeutic home services.

Position Summary:

The position is responsible for primary administrative and clinical supervision of the REACH Clinical Team. Position is responsible for shared rotation of a 24/7/365 crisis line which includes on-site response. This position is indirectly supervised by the Clinical Director and the Regional Director.

Essential Job Functions:

  • Responsible for the implementation of REACH Clinical Team Services (including, but not limited to telephonic and on-site crisis intervention, crisis prevention, comprehensive service evaluations, emergency assessments, systemic consultation, education and training to systems of care, facilitating cross systems crisis prevention and intervention planning, etc.) and other supports for individuals with intellectual/developmental disabilities and co-occurring mental illness or behavioral health needs.
  • Responsible for ensuring REACH clinical team services are provided in accordance with fidelity to the REACH model as described in literature published by the National Center for REACH Services at the University of New Hampshire Institute on Disability and the Commonwealth of Virginia DBHDS.
  • Responsible for reviewing staff service documentation for clinical appropriateness and accuracy.
  • Responsible for coordinating claims submittal and revenue collection from primary payor sources (e.g. third party payments).
  • Responsible for retrospective service review to ensure services provided were in accordance with local, state and federal requirements and with fidelity to the REACH model.
  • Responsible for ensuring Regional REACH clinical team services maintain appropriate state DBHDS licensing approval.
  • Provides clinical and administrative supervision of REACH coordinators.
  • Responsible for the implementation of REACH supports for individuals with intellectual/developmental disabilities and co-occurring mental illness or behavioral health needs.
  • Maintains linkages and relationships with community partners.
  • Ensures the coordination of support meetings and cross systems crisis prevention and intervention plans for individuals supported by REACH.
  • Provides 24/7/365 response to crisis situations according to on-call schedule.
  • Participates in required meetings with Team Members, REACH Director, Crisis Therapeutic Home staff and other applicable parties.
  • Completes Comprehensive Service Evaluations.
  • Coordinates individuals? care with identified Case Manager or other Clinical Home Provider.
  • Coordinates individuals? care with appropriate CSB representatives.
  • Coordinates individuals? care with applicable crisis responder (Emergency Services, Mobile Crisis Teams, Law Enforcement, Hospital Emergency Departments, Walk-in Crisis, etc.).
  • Identifies and coordinates training and technical assistance needs for providers, natural supports and other components of the system of care.
  • Coordinates admissions/discharges and requests for in-home crisis supports for appropriate individuals to/from REACH CTH programs in collaboration with the REACH CTH Director and REACH Director.
  • Works closely with State Psychiatric Hospitals and Training Centers for support of individuals transitioning to the community.
  • Collects and enters required data and produces documentation on administrative and service delivery and overall utilization of REACH services.
  • Other duties as assigned/required.

Supervisory Responsibility:

  • Direct supervision of REACH Coordinators
