Discharge Planner I
As a member of the PEMHS treatment team, you will complete individualized assessments, treatment planning, case management, and discharge planning for consumers during their stay. Compliance with all documentation requirements is mandatory.
Education: Bachelor’s Degree in Social Work or in Human Service field
Experience: One year of experience as mental health social worker preferred or with one year of relevant work experience acceptable.
Special Requirements: Exposure to consumers; treatment program conditions, high turnover in consumer census with specific paperwork on each consumer.
