General Description: The Family Housing Case Manager – Rapid Re-housing (RRH) performs a full range of housing-focused, client-centered case management services with families and single women experiencing homelessness, so that they can achieve housing stability. The Family Housing Case Manager primarily provides community intakes, case management, community-based housing stabilization services, and discharge planning for households referred to Housing Families First’s
Building Neighbors Rapid Re-housing program through the region’s Coordinated Entry process. This position reports to the Program Director, with some additional day-to-day site supervision by the
Rapid Re-housing Manager.

Hours: 40 per week Classification: Exempt

Duties include but are not limited to the following:

  • Serves as contact for referrals from Greater Richmond Continuum of Care (GRCoC) access points.
  • Provides intake and orientation for program participants.
  • Establishes supportive, respectful, trusting relationships with persons with histories of homelessness, severe and persistent mental illnesses, trauma, and/or substance use disorders.
  • Remains culturally sensitive to the social, psychological, and healthcare needs of the population.
  • Endures commitment to the right and ability of each person, including those with severe and persistent mental health illness, to live in normal community residences; work in market jobs; and have access to helpful, adequate, competent community support services.
  • Partners with GRCoC Outreach workers, as needed, for client engagement in the field to promote client connectivity to services and present possible housing interventions.
  • Conducts comprehensive in-depth assessments utilizing the intake eligibility, VIFSPDAT assessment, and housing barrier tools.
  • Partners with participants to prepare and implement individualized housing stabilization goals and plans while assisting participants to obtain necessary documentation.
  • Engages in housing counseling through housing search coaching, landlord navigation.
  • Determines appropriateness of financial assistance funds and recommends amounts.
  • Carries a minimum caseload of 20 families and/or single women and conducts home visits* to ensure housing retention and improvement of health and well-being.
  • Conducts reassessments of housing goals and plans in partnership with participants at least monthly. Focuses on client-centered service planning while coaching on creative ways to increase livable income and building confidence in money management through incremental self-sufficiency models.
  • Works closely with the RRH Community Housing Navigator to recruit and maintain relationships with housing owners and landlords to connect families to housing, negotiate housing application approvals and lease fees, perform required unit inspections, help avoid client evictions, and/or relocate or transition the client into a secondary housing intervention.
  • Coordinates creative use of mainstream benefits and formal and informal community-based services, vocational rehabilitation and other employment services to support participants’ long-term housing stability.
  • Works collaboratively with agencies within the GRCoC and attends meetings.
  • Uses technology, including the Homeward Community Information System database, and hard copy files to maintain, update, and report data and outcomes in a timely manner.
  • Promptly and professionally addresses and diffuses crisis situations with participants. Assesses and appropriately addresses crisis/safety issues.
  • Serves on the agency emergency on-call rotation.
  • Occasionally drive clients to leasing appointments.*

Required Knowledge, Skills and Abilities:

  • Values and practices the mission of Housing Families First.
  • Bachelor’s degree in Social Work/Human Services field required or equivalent knowledge, skills and abilities; at least two years of experience in case management and/or homeless services required. Housing Counseling experience desired. Permanent Supportive Housing program experience preferred.
  • Strong knowledge of the complexity of homelessness and co-morbidities, including mental illness, trauma, substance abuse, aging, domestic violence, HIV/AIDS related issues, and chronic health issues, as well as factors that negatively impact low-income and multi-ethnic communities.
  • Ability to efficiently work in a team, as well as independently, in an environment with minimal supervision whether remotely or on site.
  • Knowledge of social services trends, homeless and housing interventions, and best practices.
  • Experience and comfort working with individuals with mental illness in a trauma-informed manner.
  • Impeccable judgment, ability to comply with grant regulations, and a very strong detail orientation
  • Ability to analyze case information and to reach sound decisions on the basis of such information.
  • Strong interpersonal, written and oral communication skills; proven ability to communicate case management notes clearly, concisely, and with relevant, diverse audiences.
  • Ability to use a computer for database management, to establish and maintain case records, and to facilitate data collection.
  • Ability to maintain professional ethics and confidentiality of client information.
  • Ability to establish and maintain effective working relationships with a variety of individuals and deliver services in a culturally competent manner.
  • Background Check clearance, valid Virginia Drivers License, and acceptable driving record.

Pay: $38,000.00 – $40,000.00 per year
