Job Overview
The Hospice Social Worker plans and evaluates the need for medical social services for patients and families seeking services. Successful outcomes of this position will be evident by timely documentation of all services. Patient satisfaction surveys to reflect overall patient and family satisfaction with Hospice and bereavement services. Anticipatory planning and excellent communication with teammates, families, and healthcare providers are well documented.
- Assesses the emotional/social factors in order to determine the patient’s capacity and potential to cope with problems of daily living.
- Works with a patient plan of care directly under physician’s orders, in conjunction with other skilled services of the agency.
- Advises nursing and ancillary personnel regarding patient’s emotional/social function level and instructs them inappropriate communication and interaction techniques.
- Helps patient’s families understand, accept and follow medical recommendations and provide services planned to restore the patient to optimum social and health adjustment within his capacity.
- Assists and advises the physician, other health team members, or ancillary personnel on the patient’s emotional/social functioning and communication and interaction techniques.
- Assists patients and their families with personal and environmental difficulties which predispose them toward illness or interfere with obtaining maximum benefits from medical care.

- Must possess a Bachelor’s Degree in Social Work, Sociology, or Psychology from a school accredited by the Council on Social Work Education. Master’s of Social Work preferred.
- Must have at least one year of social work experience in a health care setting.
- Must demonstrate maturity, initiative, tact, and good judgment in dealing with patients and families
- All staff is required to have a valid driver’s license, reliable vehicle, and current proof of auto insurance if using their car for Agency business.
- Current CPR card.
- Proof of TB screening.
- Proof of COVID-19 vaccination, or have a bona fide medical or religious exemption.
