Job Overview

Position Purpose

With the direction of the Director of Housing Services and guidance of the Housing Programs Manager, the Housing Case Manager will provide housing stability services to HIV positive individuals and persons considered at high risk of HIV with a focus on the Homeless population and homeless prevention. Housing stability services will include case management across the continuum of housing stability services, provision of financial resources, and referrals to collaborative agencies and others. The Case Manager is responsible for maintaining accurate financial assistance records, service delivery records, and evaluation and reporting requirements in HMIS.

Scope of Responsibilities

Essential Functions

  • Assist in determining client eligibility for housing services and other community resources.
  • Stay up to date on agency eligibility policy, proper eligibility documentation requirements, and use of agency forms.
  • Collect eligibility documents from clients that fit current agency requirements and assist clients in obtaining or providing such documents.
  • Conduct phone and in-person screenings to assess potential clients for services, as needed.
  • Conduct client intake appointments to obtain client-centered information relevant to the housing situation and social service needs of the client, as needed.
  • Conduct eligibility re-certifications of current clients per programmatic policies, as needed.
  • Provide case management services, information, and referrals for TBRA clients, with a focus on housing stability.
  • Work with clients to develop housing plans that focus on individualized immediate and long-term goals with emphasis on housing stability.
  • Process financial assistance requests in line with client’s budgeting and housing plan goals.
  • Advocate on behalf of clients to address housing and other service barriers and refer clients to other community resources when appropriate.
  • Provide timely and accurate referrals to community AIDS services organizations, community resources, and medical providers as appropriate.
  • Conduct routine follow-up to reassess needs and ensure linkage to services.
  • Maintain TBRA client hard files, including eligibility/enrollment paperwork, case notes, individual service plans, financial assistance, assessments, and other items as specified on file checklist.
  • Manage case data by entering client data in Homeless Management Information System (HMIS) Service Point web-based database in a timely manner.
  • Maintain client record documentation in Provide Enterprise according to agency standards.
  • Provide information about housing programs and referrals to services for clients and community members.
  • Stay up to date on resource availability of community housing programs.
  • Explain and enforce HOPWA rules and regulations when working with clients, case managers and landlords.
  • Build and maintain working relationships with landlords and property managers.
  • Attend and participate in Department meetings as scheduled.
  • Participate in the ECHO’s Landlord Outreach and attend relevant ECHO meetings as needed. Participate in other housing and collaborative meetings for service coordination.
  • Participate in agency Quality Improvement activities as indicated.
  • Participate in the Point in Time Count annually
  • Perform other duties as assigned by the Director of Housing Services and the Housing Programs Manager.

Required* Experience and Skills*

  • Bachelor’s degree in social work, psychology, counseling, or related field required
  • Two years of experience working with individuals of diverse cultures, ethnicities, socioeconomic backgrounds, sexual orientations, and gender identifications required
  • One year of experience working with clients with substance use and/or mental health disorders and with other vulnerable populations required
  • Ability to maintain safe driving record, vehicle liability insurance and current Texas driver license
  • Ability to perform routine walking, standing, bending, lifting, and stooping during the course of day
  • An Influenza vaccine is required of all new employees at the time of employment, and is required thereafter on an annual basis.
  • A tuberculin/TB skin test is required of all new employees at the time of employment, and may be required thereafter on an annual basis, depending on position.

Additional Preferred* Experience*

  • One year of experience in working with homelessness prevention and/or the homeless population preferred Training in harm reduction principles and motivational interviewing skills preferred
  • Knowledge of HIV, homelessness, mental health, and substance abuse
  • Knowledge of diverse populations and community resources

  • Bilingual/ bicultural (English and Spanish) skills preferred
  • Ability to make sound decisions in accordance with agency policies, procedures, and guidelines
  • Ability to demonstrate good judgment, maintain strict confidentiality standards, and adhere to professional standards as defined by state and federal regulations
  • Ability to take all precautionary steps to ensure the protection and integrity of protected health information
  • Ability to work comfortably with diverse populations, with sensitivity to issues concerning HIV and all disabilities
  • Ability to prioritize multiple tasks and competing priorities
  • Ability to establish and maintain good working relationship with coworkers
  • Ability to communicate effectively, both orally and in writing
  • Ability to maintain composure when under stress and skill in resolving conflicts
  • Skill in operating office equipment, such as personal computer, calculator, copy machine, facsimile machine, and telephone system
  • Skill in using various software, including word processing, spreadsheet, and database programs
