Location: 100% Remote
Hours: Full time (W2) Pacific Standard Time 40 hours per week within the range 10-6pm Sat-Sun and 2-8pm Mon-Fri
Agno Health is a new virtual behavioral health services company on a mission to expand screening and solutions-based treatment for the increasing number of Americans with anxiety, depression, and addiction. We work in a collaborative care model with primary, urgent, and specialty medical care providers, to integrate rapid, effective behavioral therapies with the whole patient treatment program.
Responsibilities:
- Conduct brief (typically <30min), evidence-based behavioral therapy over video conference, for assigned caseload of individuals
- Welcome patient to the Agno program after a “warm handoff” from originating medical provider, describing the services offered by Agno in conjunction with the standards of the Collaborative Care model (training will be provided)
- Assess root causes of PHQ9/GAD7 screened depression or anxiety
- Develop effective, short-term treatment plans, using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate
- Engage frequently with patients through HIPAA-compliant communication/tools, with exceptional interpersonal skills and professionalism
- Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment
- Maintain appropriate records, to be shared and discussed transparently with care team including primary care provider and psychiatric consultant
- Ensure relevant data/medical records are securely transferred into the Agno EHR system for follow-up after “warm handoff” from originating clinic
- Document regular assessments, care plan updates, visit notes, and all encounters in Agno EHR, with strict adherence to HIPAA and confidentiality policies
- Assist patients with scheduling follow-up behavioral health appointments
- Monitor data on patient progress in Agno Registry, prioritizing urgent collaboration with psychiatric consultant and/or primary care provider on high-risk or non-improving patients
- Attempt to discharge patients as rapidly as they are ready, based on healthy PHQ9/GAD7 screen scores
- If not showing significant improvement within 6 months, work with the care team to refer to a more specialized mental health provider
Basic Qualifications
- Licensed or working toward a license in Clinical Social Work (LCSW or similar) in California, Texas, Washington, Tennessee, Georgia, or Massachusetts (Agno can provide LCSW supervision to support licensing process)
- Prior tele-health and/or collaborative care experience a plus
- Experience practicing behavior therapy techniques including Cognitive Behavioral Therapy (CBT), Behavior Activation, Motivational Interviewing, Dialectical Behavior Therapy, and/or Problem Solving Therapy (additional Agno training to be provided)
- Experience working with both Adults and Adolescents
- Comfort with medical record management, scheduling and billing (additional Agno training to be provided)
- Passion for our mission to expand access to effective behavioral health solutions for all who need it, and dedication to highest quality and efficiency patient care
- Familiarity with health and human services for referrals is a plus
- Exceptional communication, active listening, and interpersonal skills amongst a highly culturally diverse team and patient base
- Additional languages are a plus
- Reliable self-starter who is able to work independently, but also seek continuous learning/improvement and contribute to a fun team culture (even while remote)
- Access to a reliable internet connection and private, quiet space for virtual patient visits during scheduled work hours
