    • Ensure relevant data/medical records are securely transferred into the Agno EHR system for follow-up after “warm handoff” from originating clinic
    • Document regular assessments, care plan updates, visit notes, and all encounters in Agno EHR, with strict adherence to HIPAA and confidentiality policies
    • Assist patients with scheduling follow-up behavioral health appointments
    • Monitor data on patient progress in Agno Registry, prioritizing urgent collaboration with psychiatric consultant and/or primary care provider on high-risk or non-improving patients
  • Attempt to discharge patients as rapidly as they are ready, based on healthy PHQ9/GAD7 screen scores
    • If not showing significant improvement within 6 months, work with the care team to refer to a more specialized mental health provider

    • Exceptional communication, active listening, and interpersonal skills amongst a highly culturally diverse team and patient base
      • Additional languages are a plus
