Jones-Harrison is currently seeking a Licensed Social Worker (LSW) to join our team! The LSW serves as a liaison between the resident, family members, staff, and community resources in promoting the residents’ physical, emotional, psycho-social, and spiritual needs.
Pay: $45,000 to $52,000 annually, based on experience
Shift: Monday through Friday (40 hours per week)
Job Duties and Responsibilities Include:
- Provide emotional support and counseling to resident and families from intake to discharge through personalized visits
- Performs psychosocial assessments, case management, education and ongoing evaluation of residents and families
- Communicate appropriate psychosocial information to the interdisciplinary team regarding resident and family needs.
- Provides social work expertise regarding changes to the patient’s level of care
Education/Experience Requirements:
- Bachelor’s Degree in Social Work
- Licensed by, and in good standing, with the MN Board of Social Work
- Minimum of one-year previous experience as a Social Worker in a skilled nursing care setting
- Knowledge of resources in the community to refer residents needing additional assistance
- Working knowledge of state and federal rules and regulations as they relate to long term care.
- Knowledge of reimbursement systems affecting residents in long term care facilities (Medical Assistance, Medicare, HMO’s)
- Must receive a “not disqualified” criminal background result from DHS and may not be on the OIG exclusions list
- In accordance with CMS Vaccine Mandate, candidate must provide proof of Covid-19 Vaccine or submit documentation for exemption
Knowledge/Skills/Abilities:
- Ability to multi-task and work efficiently independently and as a team in a fast-paced environment
- Must have good people skills, ability to work cooperatively with staff, families, residents, and administration for the benefit of the resident
- Must have the ability to read, write, and follow oral and written directions at a level necessary to accomplish the job.
- Ability to work with residents and staff from diverse backgrounds
