Job Overview

The Children’s Home of Pittsburgh is seeking a Full-Time Medical Social Worker for our Pediatric Specialty Hospital *.

This position will provide social services to the Pediatric Specialty Hospital patients and their families based on an individualized assessment of the social, psychological, and economic needs. To assist families of hospitalized patients with supports and resources throughout hospitalization with the goal of a safe discharge home. Will also serve Child’s Way patients and families on an as-needed basis at the discretion of the Chief Operating Officer.

Essential Functions
1. Obtain relevant psycho-social review by meeting with appropriate families to assess, provide support, and interaction. Provide direction to the professional personnel engaged in discharges, patient advocacy, and community referrals.
2. Participate as a member of the hospital team in defining problems and in establishing a plan of care upon admission and in preparation for discharge.
3. Document progress toward goals, results and relevant information in the progress notes.
4. Participate in the Hospital Administrative Team.
5. Remain abreast of advances in the field of Social Work, legislation pertinent to departmental services and community programs/services; adapt departmental services accordingly.
6. Attend meetings, interagency conferences, etc., in order to ensure professional development and growth.
7. Responsible for the administrative casework and counseling services, and other related progress. Monitor social work assessment, treatment, and referral services, including recommending appropriate referrals and follow-up care by community agencies and support systems.
8. Provide therapeutic intervention for families through individual, family and marital counseling in collaboration with the other members of the hospital team.
9. Facilitate the exchange of information between the family and the hospital team.
10. Establish a network of peers within the referral hospitals and community resources for the flow of information and the continuity of care.
11. Participate in team meetings for the families of patients as needed.
12. Assess the need for a team meeting and setup with family and team.
13. Conduct case management conferences on an as needed basis.
14. Available for administrative on-call responsibilities.
15. Complete psychosocial assessment within 4 days of admission.
16. Maintain social services manual according to all federal and state regulations.

17. Perform all other duties as assigned.

Physical Requirements1. Partially active and partially sedentary position, must be physically able to access the patients and families, and also must be able to sit for long periods of time.
2. Must be able to push, pull, lift, and carry up to 10-50 pounds as required to access file boxes, supplies, etc.
3. Ability to interact and communicate in a professional and courteous manner with other staff and guests.
4. Manual dexterity necessary to operate computer and telephone systems, and other typical office equipment.

5. Patient care environment.

Minimum Skills and Abilities Required
1. Level of knowledge equivalent to that ordinarily acquired through the completion of a Masters Degree in Social Work.
2. Minimum of two years of social work experience required to have acquired and learned how to apply social work skills and will ensure familiarity with local agencies and facilities engaged in, or affiliated with, social services.
3. Maintains current certification as a Licensed Social Worker (LSW).
4. Act 33 and 34 Clearance.
5. Capacity to foresee potential problems, and alleviate them by utilizing available resources and providing appropriate counseling skills.
6. Interpersonal skills necessary to assess patient/family needs, occasionally provide social and psychological counseling to patients and families, interact with all levels of hospital personnel, and communicate/cooperate with numerous outside agencies
