Job Summary And Qualifications

As a Medical Social Worker within the Emergency Department, you will assist the patient/family to achieve an effective transition from hospital to post-hospital care through psychosocial evaluation, counseling, information and referral, patient education, and assistance with discharge planning. You will assist with policies and procedures, record keeping and reporting, performance improvement, and interfacing with other hospital units, community service agencies, chemical dependency and psychiatric treatment facilities. You will act as a liaison between the facility and resources external to the organization.

  • You will perform a comprehensive assessment of psychosocial needs of assigned patients
  • You will monitor and revise plans of care in collaboration with the interdisciplinary team
  • You will assess patients’ discharge needs and facilitate the provision of services necessary to meet identified needs
  • You will develop an individual plan of care for recurring patients to include education related to accessing healthcare services at the appropriate level of care
  • You will provide education to the under-resourced patient or family of potential and available resources

Experience

  • Three years of Social Work experience
  • Hospital and/or acute care case management experience – preferred

Education

  • Master’s Degree in Social Work from a school of Social Work accredited by the Counsel of Social Work Education

Licenses and Certifications

  • Current Texas licensure, acceptable licensures include Licensed Baccalaureate Social Worker (LBSW), Certified Master Social Worker (CMSW), Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW)
  • Basic Life Support (BLS) certification as required per St David’s Healthcare policy – preferred
  • Certification in Social Work or Case Management, Licensed Professional Counselor (LPC) – preferred

St. David’s North Austin Medical Center is a multi-specialty hospital in Texas with more than 360 beds. The facility is home to St. David’s Children’s Hospital, St. David’s Women’s Center of Texas featuring a Level III NICU, a Kidney Transplant Center, and the Texas Institute for Robotic Surgery. Additional services offered at this medical center include 24-hour emergency care, heart and vascular care, neurology and neurosurgery, rehabilitation services, and inpatient and outpatient surgery.
