Job Overview
We are seeking qualified candidates to provide case management services to children, adolescents, and adults affected with Mental Health Disorders, and/ or At-Risk Behavior throughout the Duval County areas. Services are rendered in the home, community, and school settings.
Responsibilities include, but aren’t limited to the following: Documentation of service delivery and activities; Coordination and oversight of initial and ongoing assessment activities; Initial development of the client Assessment, and Service Planning; Assist the recipient in acquiring mental health treatment and substance abuse recovery skills and resources necessary to successfully address his/her mental health state, medical, educational, vocational and housing needs, etc.; Collaborate with external providers and include their input into the Service Plan process; Assume the role of advocate, liason, coordinator and monitor of the service delivery system on behalf of the recipient; Complete all required documentation to ensure that there is not a lapse in service delivery and/or service authorization; Complete referrals, screenings, and intakes as needed; Conduct home visits and provide on-going support and service monitoring to ensure that recipients are receiving adequate services and assistance to meet their needs; Meet deadlines in a timely manner, and Maintain a caseload of up to 40 clients.
Candidates will also be required to become Board Certified Case Managers within the first 90 days upon hire. Education- Bachelors Degree in Human Services, Psychology, or related field, and two-years case management experience, and serving children and/ or adults within the specified population. Bilingual Candidates a plus.
