Overview:

Provides case management/therapy for patients and families to enhance their psychosocial

functioning. Performs duties and responsibilities in a manner consistent with mission, values and Mercy Service Standards.

Safety/Risk Management:

  • Follows confidentiality policy with regard to patient information spoken, written or exchanged with others
  • Complies with Infection Control guidelines.
  • Complies with patient rights and regulations.
    Adheres to hospital-wide and department safety regulations.

Coordination of Services:

  • Serves as advocate for the patient/family to the Interdisciplinary Team and encourages patient/family participation in the plan of treatment and discharge plan.
  • Works with the patient/family member, the Interdisciplinary Team members, Nursing, Rehab, Dietary, etc. and physicians to plan for the patients discharge and aftercare.
  • Participates in QI activities that identify ways to improve patient care.
  • Is knowledgeable of community resources and utilizes resources wisely.
  • Makes sure arrangements for patient discharge are appropriately and safely carried out.

Basic Aspects:

  • Performs case management/therapy for behavioral health services. Assures that the related emotional and social needs of the patient are met and maintained on an individual basis.
  • Completes initial assessments on each patient to include demographic and psychosocial information. Assures information is documented in medical record for each discipline to use.
  • Meets with patients and/or family members to develop and review treatment plan goals and prepare for discharge. Obtains appropriate signatures.
  • Provides crisis intervention, individual/family/group therapy according to patient needs.
  • Appropriately assess and intervenes in situations of abuse or neglect. Takes appropriate action as mandated reporter.
  • Assists patients and family members with discharge planning. Provides information and/or referrals for medication management, psychiatry, primary care physician, support/recovery meetings, therapy, and other resources as needed.
  • Monitors ongoing needs of patient for psychosocial interventions and appropriate discharge plan.
  • Completes required documentation on clinical record. Attends interdisciplinary team meetings and provides input on patient status and discharge plan.
  • Updates and maintains care plan/treatment plan to reflect changing needs of patients utilizing appropriate resources and documents patients and/or family response to education.
  • Keeps abreast and complies with regulatory changes. Participates in annual review and revision of policies and procedures according to JCAHO standards.
  • Provides community resource information to patients and families.

Communication Management:

  • Uses education and expertise to act as resource for Social Worker I position for psychosocial counseling.
  • Provides support and education to patients and families to facilitate and adaptation to illness or disability and compliance with recommended treatment.
  • Communicates with patients and families in a timely manner in regard to patient’s progress, needs, and discharge plans.
  • Develops and maintains relationships with community agencies.
  • Communicates patient goals and changes in status to other Interdisciplinary Care Plan Team members and utilizes input to revise care plan for optimum patient benefit.
  • Makes independent decisions in routine and complex situations and informs appropriate persons of actions and results.
  • Gives thorough information to leader and/or case manager/therapist covering on weekends and during PTO to enable the staff member to easily take over in the patient’s care.

Technology:

  • Maintains knowledge base through continuing education and applies knowledge to current social work practice.
  • Working knowledge of Microsoft Office products and proficient computer skills.

Qualifications:

Education: Master’s degree required in Social Work, Counseling or other human-service related field.
Licensure: Licensed Masters Social Worker (LMSW), Provisional Licensed Professional Counselor (PLPC) or eligibility for licensure preferred.
Experience:
Preferred Experience: Minimum one year Behavioral Health experience preferred.
Preferred Other: Co-worker must be able to communicate orally, in writing and demonstrate proficient computer skills. Co-worker must be able to listen and to understand and explain complex situations.
