Job Overview
- Provide (daily) short term individual and group counseling (and psychoeducation) to youth experiencing homelessness to address mental health and psychosocial needs (per service plan). This includes short-term crisis intervention, including advocacy and follow-up for designated caseload (Case Manager/SW teams) and emergencies.
- Complete intake and psychosocial assessments (e.g., screening for mental health and substance use concerns and mental status) for higher need residents. Identify current mental health, social, and emotional needs, making initial recommendations, in-house and outside referrals and write appropriate summary reports.
- Have a full understanding of floor flow of services including Case Management
- Actively link young people who are interested in longer term mental health care and psycho-social supports to community based providers and communicate with providers/social workers at referring agencies as necessary.
- Work in small teams with assigned Case Managers to coordinate and collaboratively address the case management and social work needs youth within respective teams.
- Develop and co-facilitate (with residential staff, RAs/CMs) innovative support/psycho-education/skills building groups related to topics such as Anger and Stress Management, Substance Abuse Prevention, Affective Regulation and Healthy Relationships.
- Co-facilitate mental health focused and supportive workshops and trainings for both young people and agency staff.
- Work collaboratively with all internal and external service providers.
- Provide consultation to Case Managers and residential staff—providing assessments, information (regarding issues around trauma, mental health and substance use), and referrals as needed.
- Document and complete all assessments and reports, including psychosocial assessments, treatment plans, session notes, referrals and follow-up in a timely manner in accordance with good professional practice and agency policy.
- Ensure that paper files are grant, regulation and government agency compliant at all times.
- Ensure that all documentation is completed in ETO and HMIS in a timely manner. Maintain ETO and HMIS records, clearly document all youth engagements and meaningful interactions using the S.O.A.P. format.
- Meet regularly with clinical supervisor to discuss concerns, reflect on practice and client needs, and carry out plans for young people.
- Hold a target of 8 cases for high needs youth
- Will serve as clinical Manager-On-Duty (MOD) on a weekly rotation including other CHGW Programs. The expectations of, and list of duties will be provided by the department, and will include on-cal evening clinical support coverage for that week, and the entire weekend.
- May serve in a resident advisor capacity to cover a shift, as needed.
- Participate in weekly case review and daily shift transition.
- Flexible schedule required, sometimes including holidays and weekends, as needed.

Qualifications:
* Master’s Level and Licensure required (social work or counseling)
- Experience providing crisis intervention, short term individual and group counseling and case work with at risk youth highly preferred. Internships in this field to be considered.
- Knowledge of trauma and mental health issues facing multi-stressed youth and families.
- Experience working in a shelter or residential setting preferred.
- Knowledge of external and social service systems.
- Solid assessment, interviewing and crisis intervention skills.
- Flexible work schedule to include evenings, weekends and holidays.
