Job purpose

The general purpose of this position is to support young adults in the Pathways HOME Program. The primary purpose of your role is to provide strengths-based case management to Pathways HOME Program clients in accordance with all current federal, state, and local standards, guidelines, and other regulations that govern the program. The Family Resource Specialist will assist clients with accessing and maintaining housing, increasing income, connecting with mainstream and community resources and natural supports, and improving independent living skills in the areas of employment, education and social skills.

Duties and responsibilities

  • Provide weekly individualized, client-centered case management to an assigned caseload. Case management visits could occur in the office or in the clients’ apartment or at another public location (such as a restaurant, library, park, etc.). Case management should be catered to the achievement of specific client goals and include a focus on housing stability. Case Management should focus on partnering with clients on securing and maintaining stable housing in the community, increasing income and building social supports.
  • Posses and build a strong working knowledge of community resources and provide warm handoffs as needed with other service providers.
  • Develop appropriate quarterly case plans that address specific client needs and provide goals to promote independence and housing stability.
  • Complete all needed client and program paperwork accurately on a monthly basis.
  • Work closely with Housing Case Manager to match clients to an appropriate unit as quickly as possible.
  • Conduct monthly visual unit inspections to ensure safety of participants and compliance with lease.
  • Complete required monthly program outcomes for all clients.
  • Facilitate and help develop bi-weekly Life Skills programming. May facilitate and lead life skills classes to/for Pathways HOME clients.
  • Provide client transportation to weekly Pathways HOME programming events and to community appointments or other locations as needed within the context of meeting case plan goals/client needs.
  • Conduct administrative chart reviews for client records as assigned.
  • Provide crisis intervention and on-call emergency support to Pathways HOME clients as needed.
  • Attend program meetings as scheduled including staff meeting, administrative and clinical supervision, and additional Pathways HOME meetings as needed.
  • Work as part of the Pathways HOME team to provide high quality services for clients and bring creative ideas to supervisors for program growth and improvement.

Qualifications

  • Bachelor’s Degree in social work, counseling or related human service field required.
  • Two years of experience providing services to children or young adults preferred.
  • Must have knowledge of normative youth development and effects on youth development of early trauma, educational gaps and delays, and abuse and neglect.
  • Must possess case work, group work, and case coordination skills.
  • Valid driver’s license and clear driving record required.
  • Must be able to read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess a willingness to work harmoniously with other personnel.
  • Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.

Working conditions

  • Works in office area(s) as well as in Pathways HOME client homes and other community settings (e.g. social services agencies, local homeless shelters, etc.).
  • Moves intermittently during working hours.
  • Is subject to frequent interruptions.
  • Is subject to hostile and emotionally upset clients, family members, personnel, and visitors.
  • May drive a 15-passenger van to transport clients.
  • Communicates with other department supervisors.
  • May work beyond normal working hours and on weekends and holidays when necessary.

Physical requirements

  • Must be able to move intermittently throughout the workday.
  • Must be able to lift 85 lbs. Must be able to lift heavy items, including furniture and household items with clients when they secure housing.
  • Must be able to cope with the mental and emotional stress of the position.
