PERSONAL SERVICES COORDINATOR - FSP-TAY

Job Overview

**Work Schedule Mon- Fri 9:30am-6:00pm; Flexible per program needs; Available for some evenings and weekends Bi-lingual Spanish or Vietnamese Speaking preferred**

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Provide integrated, recovery-oriented, direct transition services to high risk and/or under-represented transition-age youth, adults, and older adults with serious mental illness, co-existing disorders, including medical conditions, substance abuse, and developmental disabilities, and with a history of high use of psychiatric services, hospitalization and long term institutional care, incarceration, and/or homelessness.*
    • Develop a service relationship to provide outreach and engagement with potential participants in a wide variety of community settings, including jails, hospitals, and institutions.
    • Ensure that all services are voluntary, provided in partnership with the participant, and driven by individual choice.
    • In collaborative partnership with each individual, develop and implement comprehensive Personal Services Plans that are consistent with the principles of wellness and recovery.
      • Offer “whatever it takes” to engage program participants in a partnership to achieve their individual wellness and recovery goals, using alternative models of care which offer greater benefits to them, increasing the likelihood that they will experience positive outcomes;
      • Ensure that each goal is derived from and agreed upon by the program’s participating partner.
    • Provide coaching/counseling in whatever areas necessary to achieve skills that allow the partners to meet and sustain the quality of life improvements, as outlined in the Personal Services Plan.
    • Utilizing a strength-based approach, increase partners’ resources in the community, relationships with family, and their peer/social networks.
    • Ensure that the delivery of all services will be guided by the principles of cultural competence, recovery, and resiliency.
  • Assist and advocate for program partners in linkage to community resources necessary to achieve Personal Services Plan goals, particularly in the areas of housing, employment, education, medication, peer relations, social activities, and relapse prevention.*
    • Assist partners to identify, cultivate and sustain relationships necessary to create a community support network.
  • Complete documentation and maintain partners’ files in accordance with program policies and funding source requirements.*
  • Provide 24/7 on-call coverage and participate in after-hours rotation, as assigned.*
  • Provide transportation in personal or agency vehicles including, but not limited to, the transportation of clients and/or program supplies for agency business.*

OTHER DUTIES AND RESPONSIBILITIES:

  • Attend and participate in staff, program, agency and community meetings, as required, requested or assigned.

KNOWLEDGE, SKILL, AND ABILITIES REQUIRED: (E = Essential; D = Desired)

  • High School diploma, GED, or equivalent education plus five years’ experience; OR Associate’s degree plus three years’ experience; OR Bachelor’s degree plus two years’ experience; OR Master’s degree in a related field plus one year’s experience, working in a human service setting. (Appropriate volunteer and internship experience and personal experience in the mental health system will be considered.) (E)
  • In some cases, Bachelor’s degree, MHRS qualifications and/or a greater number of years of experience (will be noted on a posting). (E)
  • A degree in a related field and experience in a mental health setting or with emotionally disturbed individuals. (D)
  • Demonstrated ability to:
    • Work independently and in a small team environment; (E)
    • Engage and communicate effectively with others in both verbal and written English; (E)
    • Monitor and support developmental growth and the recovery process; (E)
    • Manage multiple tasks and demands; solve problems; (E)
    • Employ tact and diplomacy, exercise good judgment, and act calmly in emergency situations; (E)
    • Work from a strengths-based, harm-reduction, social rehabilitation model; (E)
    • Work effectively with persons differing in value systems, ethnicity, cultural backgrounds, language capabilities and disabilities; (E)
    • Work collaboratively with individuals identified as those with serious mental illness who meet the following criteria: histories of hospitalization, institutionalization, substance abuse, low engagement in medication treatment, and difficulty in participating in structured activities and living independently; (E)
    • Work with populations identified as high risk and/or considered under-represented in the County Mental Health system, with multiple barriers; (D)
    • Exhibit exemplary customer service, compassion, and care in the execution of all duties and interactions. (E)
  • Demonstrated ability to work with participants who:
    • May also have histories of assaultive behavior; (E)
    • May have resided in long term care facilities for extended periods; (E)
    • May have patterns of service that relied almost exclusively on emergency and institutional care; (E)
    • May have cognitive difficulties and medical co-morbidities and are considered medically fragile and may also be developmentally disabled; (E)
    • May have long-term patterns of homelessness and criminal justice involvement. (E)
  • Knowledge of:
    • Specific needs of the target population, as well as serious mental illness, substance abuse, other dual diagnoses, homelessness and the justice system; (D).
    • Mental Health Services Act (MHSA) and Wellness Recovery Action Plan (WRAP); (D)
    • Clinical techniques for assessment, treatment and rehabilitation of people with mental illness; (E)
    • Community resources, including those required to meet the non-mental health needs of the target population. (D)
  • Proven proficiency in typing—minimum 30 net wpm; computer literacy, including Internet search and a basic proficiency level in Microsoft Office applications and Windows-based databases; (E) familiarity with UNIX-based databases. (D)

  • Possess a valid California Driver License, with access to a reliable vehicle or for driving an Agency vehicle, current proof of auto liability insurance, and a documented clean DMV record. (E)
  • Physical requirements needed to perform the essential functions of this job in day-to-day performance of the duties of this position, with or without accommodation:
    • Hearing and talking on telephone and in person; standing, walking, sitting, pinching and finger flexion are required constantly (over 2/3 of the workday); (E)
    • Lifting, carrying, pushing, pulling, bending, stooping, crouching, kneeling and twisting are required occasionally (under 1/3 of the workday); (E)
    • Balancing, climbing and crawling are generally not required.
