SUMMARY:
 The Program Coordinator/Manager assists with all aspects of program development and implementation to ensure quality of care in accordance with Department of Mental Health licensure requirements. The Program Coordinator/Manager supervises Companion at the companion sites, Host at the Host/Foster Family home, and Site Supervisors at ISL’s.

General Expectations:

a. All current customer and employee documents must be kept in a secure manner and be available for review at a moment’s notice.

b. The Individual Support Plan Tracking (including all components such as the Behavior Support Plan, the Budget Information, Due Process form must be submitted to the Program Director assigned as the Program Coordinator/Manager’s supervisor by the 5th of the month.

c. Add supporting information to Therap regarding customer care. (i.e. Outcome and Goals attaching the current ISP and BSP, treatment(s), Monthly Narrative that is consistent with the goals, etc.)

d. Email Case Manager monthly narrative, RN summary, drills, and water temp by the 15th of each month.

e. Provide staff in the Companion or Host model with Therap access prior to working.

f. Reviews and approved site mileage.

g. Review and approve site vehicle mileage.

h. Approve activity calendar.

i. Schedule doctor appointments and follow up to ensure progress of the appointment.

j. Communicate any customer changes to the Program Director immediately or within 24 hours. Provide ongoing updates to the Program Director regarding key issues with assigned customers/sites for the completion of the week.

k. Achieve and remain current with the following training requirements: CPR/First Aid, MANDT, Med Level 1, Orientation, Advance MANDT, Advance Support Training and any other trainings provided to enhance work productivity.

l. Complete side by side training with those you supervise.

m. Complete Program Weekly Checklist and submit to your Program Director.

n. Work closely with the Site Supervisor and Site Supervisor Coordinator for specific site needs and trainings.

o. All phone calls, text messages, and emails must be returned within 24 hours and/or next business day unless you are on-call.

p. Sign the ISP consent page within 24 hours of receiving it and submit to the Case Manager.

q. Respond and submit the action plan for Health Identification Planning System (HIPS) within 2 weeks of receiving it.

. Requirements:Must have at least 3 years’ experience in the field of developmental disabilities and 3 years supervisory experience. Experience in a related field may be considered.
