Psychiatric Services Intake Coordinator
Site Location: West Salem Clinic | 1233 Edgewater St NW, Salem, Oregon 97304
Position Status: Full Time | Monday through Thursday

YOUR ROLE at NWHS:
Facilitate access to Psychiatry services for patients of the West Salem Medical Clinic and Total Health Community Clinic. Interfaces with a multi-disciplinary care team including Primary Care Providers, Behaviorist Health Care Consultants, and Mental Health Counselors. Works directly with the Clinics Operations Officer to develop policy and workflow and the Psychiatry Services Medical Director or delegate to perform preliminary screenings, initial assessments, and formulate treatment recommendations.
DUTIES:
- Facilitates access to Psychiatric Services by being an open and collaborative main contact point for colleagues. Is available in real time to trouble shoot, problem solve, and facilitate the progression of care in acute situations that arise within the primary care clinic as it relates to psychiatric services.
- Serves as primary point of access for all referrals into psychiatric care. Reviews all referrals for clinical appropriateness, collects historical data, and consults with care team member to coordinate, prioritize, and expedite intake. Organizes and facilitates the intake triage process of new referrals with the Psychiatric Services Medical Director or their delegate.
- Administratively supports the department by securing previous records, providing outreach efforts to ensure patients complete and return intake paperwork, reviewing and writing policy, and managing statistical data to guide department decisions.
- Oversees Mental Health Cohort in PreManage to ensure regulatory access timelines post hospitalization are met and to provide follow up care post Emergency Department utilization.
QUALIFICATIONS:
- Master’s degree in a mental health discipline or higher and at least two years of work experience, preferably in a psychiatric setting. Preference goes to those with a mental health license, actively working towards licensure, or eligible for licensure in the State of Oregon.
- Must be a collaborative teammate with a passion for serving the underserved.
- Must have experience working with persons who have mental health issues and those who are in emotional distress.
- Previous experience working in an integrated Patient Centered Medical Home is preferred.
