We currently operate four Rural Health Clinics; in Oakridge, Blue River, Veneta/Elmira and Estacada, Oregon, where we provide comprehensive primary, preventative, behavioral health, and dental care services to approximately 5,000 patients. This position will spend time working as a School-Based Behavioral Health Clinician onsite at Oakridge High School in Oakridge where you will play an essential role in furthering our vision of Healthy Rural Communities and Healthcare Revolutionized by providing compassionate care to those in need in our communities.

Responsibilities

  • Provide short-term, solution focused therapy (generally in the range of 2 to 8 sessions) for patients with low to mid-acuity behavioral health needs. Common examples of such needs include: adjustment disorders, insomnia, lifestyle improvement, smoking cessation, compliance with medical treatment recommendations, and short-term behavioral skill-building.
  • Stabilization for patients in crisis: same day access and ability to deliver frequent services over a 4 wk period.
  • Care management services for patients with complex health needs being driven by their mental and/or behavioral health situation.
  • Assessment and referral to community partners for ongoing mental health needs.
  • Coordinate with outside agencies as needed (DHS, primary care, CDRC, etc.).
  • Provide training for school staff.
  • Attend school meetings and coordination of care for IEPs.Facilitates a “shared goal model” within and across settings to achieve coordinated high-quality care that is patient- and family-centered.
  • Consult and collaborate on a multidisciplinary team. This includes providing integrated on-site care, recommendations, and feedback to medical providers and allied staff.
  • Participate in our complex care management team.
  • Other duties as assigned.

Qualifications/Experience

  • LCSW, LPC, or LPCI (must be registered with the state but can be at any point in their supervision hours) required
  • Passionate about working with kids.
  • 4 days/week working at the school.
  • 50% of scheduled time is available for warm-handoffs, care management, admin time, school or Orchid staff meetings, or other population health activities and 50% of scheduled time is booked via scheduled appointments.

Compensation

  • $70k to $80k/year starting salary (prorated based on a 30-40 hour work week)

  • Generous sign-on and retention bonus: $8000 sign-on bonus, $8000 retention bonus paid at 1 year anniversary, $4000 paid at 2 year anniversary.
