Job Overview
- Plans and coordinates functions and activities related to meeting the psychological, social and nursing needs of patients and their families.
- Supports the case manager and social worker with an emphasis on discharge planning and/or maintaining medically, frail individuals in the community.
- Performs duties in accordance with all regulatory requirements as well as policies and procedures of The Queen’s Medical Center (QMC).
II. TYPICAL PHYSICAL DEMANDS:
- Essential: sitting, finger dexterity, seeing, hearing, speaking, frequent gripping of an object.
- Continuous: static gripping of an object for prolonged periods.
- Frequent: walking, lifting usual weight of 3 pounds; carrying usual weight of 2 pounds; reaching at shoulder level; repetitive arm/hand motions.
- Occasional: standing, stooping/bending, kneeling, climbing stairs, walking on uneven ground, squatting, twisting body, lifting maximum of 10 pounds, reaching above and below shoulder level.
- Operates typewriter, calculator, computer, printer, telephone, and facsimile.
III. TYPICAL WORKING CONDITIONS:
- Not substantially subjected to adverse environmental conditions.
- Subjected to noise and/or vibration due to shared office space, air conditioning and construction work in neighboring department.
- May be exposed to hostile, disruptive, assaultive and inappropriate patient behavior.
- Assignment to Clinical Care Coordination: must have access to motor vehicle.
IV. MINIMUM QUALIFICATIONS:
A. EDUCATION/CERTIFICATION AND LICENSURE:
- Bachelor’s degree in social work related field; or a bachelor’s degree in non-related field with two (2) years of experience working in a comparable job demonstrating knowledge of governmental and social agencies and services may be substituted for degree in social work or related field.
Assignment to Clinical Care Coordination (CCC):
- Valid U.S. driver’s license required in accordance with Hawaii state law rules and regulations.
- Driver’s abstract or evidence of good driving record may be required based on area of assignment.
B. EXPERIENCE:
- One (1) year experience in a health related field or social service agency preferred.
