Bartlett Regional Hospital is searching for candidates interested in a dynamic social work opportunity in health care. Case Management is a department that supports all areas of the hospital, including Acute Care, Acute Mental Health, addiction treatment at Rainforest Recovery, and the Emergency Department. Case Management is comprised of nurses and social workers that work together to provide coordinated care for our patients.

 

The current recruitment is for case management services in the Emergency Department, but will be oriented to and provide service in other settings as well. Social Work Case Managers generally have a primary location, but are expected to cross train to other areas and may be required to work in other units, based on organizational need.

 

This role is in a fast paced environment and is responsive to multiple teams and partners. Beyond the direct patient care, the case managers are involved with identifying programs, services, and projects to support patient and community needs. They participate with community coalitions to collaborate and support coordination of care and improvement in services.

Minimum Qualifications for the Job:
  • Social Work Case Manager I ($32.44 – $38.16/hour)
    • Education: Three (3) years of postsecondary education within a social work program. *Must graduate from an accredited social work program within one (1) year of hire. Desirable Experience: Experience in hospital case management or healthcare.
    • License: Must obtain Alaska Social Work License within one (1) year of graduation from an accredited social work program.
    • Certification: TEAM/BERT Certification within 90 days of hire.
    • Career Ladder: The incumbent is required to advance to the Social Work Case Manager II level once they obtain licensure and the required experience to meet SWCM II minimum qualifications. Advancement to the SWCM II level is a condition of employment.
  • Social Work Case Manager II ($35.94 – $42.28/hour)
    • Education: Graduate of an accredited social work program. Completes annual educational requirements.
    • License: Alaska Social Work License at time of appointment or within 90 days of hire if licensed in another state.
    • Certification: TEAM/BERT Certification within 90 days of hire.
    • Experience: One (1) year of experience working as a social worker in the area of health care, behavioral health, or substance abuse treatment.
  • Social Work Case Manager III ($38.12 – $44.85/hour)
    • Education: Master’s Degree in Social Work.
    • Experience: Two (2) years of experience working as a social worker in the area of health care, behavioral health, or substance abuse treatment.
    • Substitution: A Bachelor’s Degree in Social Work and four (4) years of experience as a social worker in the area of healthcare, behavioral health, or substance abuse treatment may substitute for the required education and experience.
    • License: Alaska Social Work License. Preferred License: LCSW.
      Certification: TEAM/BERT Certification within 90 days of hire. Desirable Certification: Accredited Case Manager.
