Job Overview

Job Summary: Provides assessment, services coordination and counseling to staff involved in client care, as well as provides direct service to clients as per social worker responsibilities.

Job Responsibilities

  • Maintain established agency policies and procedures, objectives, quality assurance program and safety standards.
  • Consult with and keeps department director informed of activities, needs, and problems related to demands.
  • Supervise Case Managers in daily operations of case placement – supervision, coordination, and/or client relations, and problem solve/resolve emergent issues.
  • Supervise social work students who have field placements at site, and keep in contact with faculty advisors as necessary.
  • Assure that staff adhere to mandates established by contract and that quality service as well as case coordination services are provided.
  • Assist in assessing the case worker’s skills and identifies specific training needs for staff.
  • Evaluate performance of staff periodically and reports to the director.
  • Supervise and directs case managers in developing written corrective action plans in ongoing case files, incident reports and in-home visit reports.
  • Attend case conferences with social work staff from other agencies and represents agency’s interests.
  • Attend community-based and DFTA meetings surrounding case management and supervision issues.
  • Attend staff meetings.
  • Review case records to ensure that services are provided according to the assessed care plan.
  • Assist in resolving difficult client issues.
  • Help client and family through individual or group conferences to understand, accept, and follow care plan recommendations.
  • Participate in planning for improving services by interpreting social factors pertinent to development of program.
  • Provide general direction and supervision to workers engaged in program activities.
  • Provide coverage for workers who are out of office, sick or on vacation. Substitutes for Director in his/her absence.
Knowledge, Skills And Requirements
  • Master’s degree in Social Work (MSW) or related field with two years of human services experience, preferably in the field of aging or Bachelor’s Degree (BASW) in Social Work or related field with five or more years of supervisory experience required.
  • NYS license and registration to practice as a Licensed or Certified Social Worker in New York State preferred.

To ensure the safety of our clients and staff, all new employees must be fully vaccinated prior to their start date. Upon acceptance of a conditional job offer, candidates who are not already vaccinated must obtain the first dose of the vaccine within three (3) calendar days of job offer and must obtain any subsequent doses in accordance with the vaccine protocol.
