Job Overview

The Social Worker is responsible for providing medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental, and psycho-social well-being.

RESPONSIBILITIES:

  • Identifies cognitive impairments, mood problems, and other psycho-social needs and follows-up as needed.
  • Schedules and takes the lead in interdisciplinary meetings with residents and their families.
  • Maintains accurate, thorough, and timely documentation.
  • Reviews Advanced Directives and provides education to residents and families.
  • Acts as a resident advocate and is familiar with residents’ rights.
  • Makes appropriate referrals to outside agencies.
  • Helps provide a smooth admission and discharge process for residents and families.
  • Acts as a liaison for ancillary services.

The Social Worker is required to have a minimum of a Bachelor’s degree in Social Work and a current Social Worker license or certification in good standing. A minimum of 1 year of experience required, preferably in a skilled nursing and rehab center. Other comparable healthcare experiences will also be considered.
