Social Worker / LCSW

Seeking a Full-Time Social Service Worker to join our team working with veterans and their spouses.

Qualifications:

  • Bachelor’s Degree in Social Services or related field required (e.g. human services, psychology, etc.) Master’s level LCSW preferred
  • Must have patience, cheerful disposition, and enthusiasm, as well as a passion to work with residents based on whatever maturity level in which they are currently functioning.
  • Computer and English language literacy required. Experience with MDS, PCC, and Microsoft Office a plus.
  • Must be able to be mobile about the large facility, i.e., going to resident rooms, meetings, etc.
  • 1 year experience working in a Long-term Care setting, with Veterans, Geriatric, and/or Dementia population is preferred but not necessary.
  • Must pass a Drug Screen & Background Check

Pay: This is a salaried position at 40 hours per week, with wage dependent on experience and education.

General Position Summary:

The Social Worker for the Oregon Veterans’ Home will direct social services case load, evaluate the social needs of each resident admitted and formulate a written care plan. Review and revise plans as necessary, in accordance with state/federal regulations. Refer residents and families to appropriate community resources as necessary. Perform discharge planning with resident and family as appropriate and assist in admission of residents by completing necessary paperwork with resident and/or family.

Essential Functions/Major Responsibilities:

  • Conduct Minimum Data Set (MDS) assessments in accordance with federal regulations.
  • Develop care plans that meet all the physical, social and emotional needs of residents.
  • Interview resident and family member upon admission and complete all necessary paperwork.
  • Identify and evaluate personal, emotional and environmental concerns that may limit residents’ quality of life.
  • Formulate care plan based upon the MDS.
  • Record continuing progress notes.
  • Maintain community resource information.
  • Assist other staff in appropriate use of community resources.
  • Participate regularly in care plan conferences with Interdisciplinary Team (IDT).
  • Provide new residents and family members with orientation to OVH.
  • Complete Social History Assessment.
  • Coordinate Discharge Plan for all residents.
  • Develop meaningful, ongoing relationships with family, friends, legal guardians and all other significant individuals who will contribute to the residents’ success and quality of life.
  • Collaborate with other disciplines within the facility to meet the needs of the residents.
  • Develop and maintain specific behavioral plans as necessary.
  • Participate in Psychotropic Medication Review.
  • Develop and maintain a close working relationship with public and private health and welfare agencies.

  • Related service organizations, such as Senior and Disabled Services staff.
  • State and VA Surveys.

Specific Job Skills:

  • Long Term Care Social Services case work.

  • Communication and marketing skills.
  • Staff development and support.

Education and Experience:

  • Master’s in Social/Human Services field (social work, psychology, sociology, special education or human services); LCSW preferred.
  • Experience in bringing together community resources to meet the diverse needs of geriatric community and their caregivers.

Job Conditions:

  • This job involves potentially aggressive residents.
  • This position primarily takes place at the Oregon Veterans’ Home in The Dalles.
  • Hours of employment are essentially 8 a.m. to 5 p.m., Monday through Friday, although flexible based and organizational and personal needs.
  • Occasional Saturday or Sunday hours required.
  • Occasional evening hours required.
  • Occasional travel required.

Essential Physical Elements:

  • Ability to stand and walk for extended periods of time.
  • Ability to sit for up to two hours at a time.
  • Able to lift and carry 10 lbs.
  • Ability to work at times with a high level of mental/visual strain due to computer input.
  • Ability to verbally articulate over the phone and in person.
  • Keyboarding abilities involving repetitive motions of hands and wrists.
