Job Overview
Roberts Health Centre is a 66 Bed Skilled Nursing facility, located in North Kingstown RI, directly across from the Train Station and North Kingstown Walmart.
The Social Worker will provide the residents with the highest practical level of physical, mental and psychosocial well-being and quality of life.
DUTIES:
- Identify, maintaining and/or improving the resident’s social, emotional, mental and psychosocial needs.
- Developing and maintaining individualized care plans to address social service needs.
- Hold meetings with residents, families, and significant others as requested and to provide support and information so as to improve quality of life.
- Participating in the planning of the resident’s admission, discharge planning back to the community.
- Participating and collaborate with the interdisciplinary care plan team to ensure treatment of the social and emotional needs of the resident.
REQUIREMENTS:
- EXPERIENCE WITH MDS 3.0
- Long term care/ sub-acute experience required
- Bachelor’s Degree in Social Work
