Social Workers serving in a variety of departments and clinics are responsible for responding to referrals from the interdisciplinary team. The social worker will complete psychosocial assessments, develop and implement a plan of care that addresses patients’ needs that promotes continuity of care, and facilitates transitions to the next level of care. The social worker will provide education, consultation, and serve as a liaison to the healthcare team regarding the patient and families’ emotional, psychological, social, financial and developmental aspects of care.

Responsibilities

  • PATIENT POPULATION – (CLINICAL ONLY) 5%
    • Engages in population appropriate communication.
    • Has knowledge of growth and development milestones and tasks.
    • Gives clear instructions to patients/family regarding treatment.
    • Involves family/guardian in the assessment, initial treatment and continuing care of the patient.
    • Identifies any physical limitations of the patient and deploys intervention when necessary.
    • Recognizes and responds appropriately to patients/families with behavioral health problems.
    • Interprets population related data and plans care appropriately.
    • Identifies and responds appropriately to different needs resulting from, unique psychological needs or those associated with religious / cultural norms. Performs treatments, administers medication or operates equipment safely.
    • Recognizes and responds to signs/symptoms of abuse or neglect.
  • Job Function: Hospital/Corporate Level Professional Development – 55%
    • The scope of the education specialist is primarily at the hospital/corporate level.
    • The Education Consultant is responsible for development, implementation and evaluation of the education plans through the use of education needs assessments as needed, guidance from interprofessional leadership and current evidence.
    • Uses critical thinking, the nursing process, principles of adult learning, teaching/learning theories and age-appropriate teaching strategies in development and implementation of hospital/corporate educational programs.
    • Responsibilities across UC Health include orientation and onboarding, preceptor support/development, coaching/mentoring, quality initiatives, hospital/corporate level annual competency assessment/validation, and student activities
    • Ensures effective learning experiences for competency assessment and validation to reflect practice changes, regulatory agency or organizational policy requirements.
    • Applies design principles and integrates technology to ensure effective learning experiences for competency assessment and validation to reflect practice changes, regulatory agency or organizational policy requirements.
    • Serves in collaboration with nursing and interprofessional leadership, the Professional Practice Organization (PPO), and other UC Health entities to implement nursing education and professional development programs.
    • Consults with, supports and develops unit level educators, preceptors, and staff.
  • Job Function: Role Model – 30%
    • Role models professional nursing and interprofessional collaborative behaviors
    • Demonstrates attributes of growth mindset that embody the mission/vision/values of UC Health as well as professional nursing practice standards.
    • Actions drive organizational level goals and initiatives from a nursing professional development perspective.
    • Possesses required competencies as outlined by the ANA Nursing Professional Development: Scope and Standards of Practice (2016). See addendum.
  • Job Function: Performance Improvement, Evidence Based Practice, Nursing Research – 10%
    • Initiates and/or supports, performance improvement, evidence-based practice and nursing research efforts to contribute to new knowledge and improvement.
    • Uses available data to monitor/evaluate performance of programs, initiatives and targeted improvement efforts (ex. HAIs, nurse sensitive clinical outcomes, HCAHPs, RN turnover, etc.)
    • Initiates and/or participates in project implementation (e.g.CDIS, Clinical Ladder, nurse Residency Program requirements)

Qualifications

Education: Master of Social Work from an accredited School of Social Work, minimum required.

License and Certification : Licensed by the State of Ohio Social Work Board (LSW, LISW, or LISW-S)

Years of experience : One-two Years equivalent experience, minimum required. Three-five years, preferred.

Required Skills and Knowledge:

Casework principles, including diagnosis, assessment, crisis intervention, and treatment planning; techniques in individual, family and group therapy; awareness of community resources, public assistance systems, Medicare/Medicaid; accepted professional and ethical standards within the existing medical community and professional organizations.
