Hospice/End of Life Experience Preferred

Must have a Master’s Degree in Social Work, LCSW Preferred

$30-35 per hour depending on experience:

POSITION SUMMARY:
Two positions are open – one full time and one part time.:
We are looking for a qualified full time Social Worker who will be an integral part of our Interdisciplinary Hospice Team and will assist the team in understanding the significant social, emotional, and cultural factors related to each patient’s terminal illness. The Social Worker conducts psychosocial assessments and evaluations and provides emotional counseling to patients/families. The Social Worker works under the direction of a physician and supervision of the Director of Patient Care Services and serves as a resource to our patients, families and our community. Must be able to utilize computers and be comfortable with EMR.:

We are also looking for a part time Social Worker to be part of the after hours team. By joining the after hours team, the Social Worker will provide explanations of hospice benefits, as well as make crisis and post-death visits.:

QUALIFICATIONS:
*Masters of Social Work Degree from a school accredited or approved by the Council on Social Work Education.:

L.C.S.W. preferred, but not required.:
*Clinical experience relevant to the psychosocial needs of patients and families.:

*Knowledge of psychosocial theories and use of psychosocial treatment techniques. Knowledge of beliefs and practices of cultures related to death and dying. Ability to work with dying patients and their families. Ability to make appropriate professional judgments. Sympathetic attitude toward hospice patient:

*Excellent oral and written communication skills. Spanish a plus:

*Valid driver’s license, and auto insurance, reliable/well maintained transportation.
