Job Overview

Job Summary: The role of the Social Worker will be to provide services to patients referred by the Medical Home Care Coordinators (MHCC’s) in the Hunterdon Medical Group Practices. The Social Worker will work in full collaboration with the MHCC’s to help address social determinants of health (transportation, finances, insurance).

Responsibilities

  • Proper documentation of all encounters with patients and services provided.
  • Refer patients to support programs within Hunterdon Healthcare System or throughout the communities we serve.
  • Remain aware of changes and/or updates to community options and programs available to patients and families.
  • Contact necessary community agencies when applicable, i.e. Adult Protective Services, Division of Child Protection & Permanency, Medicaid, Probation, Surrogate Courts, etc.
  • Pursue professional development and stay current with industry trends and changes.
  • Assist with special projects as assigned.

  • Demonstrates a sense of urgency and responsibility to consistently provide superior service.
  • Works only within the scope of their practice and refers all medical/ nursing issues and interventions back to the MHCC or Primary Care Team.

Qualifications

Education/Experience:

  • BSW Required
  • Knowledge of the community resources available in each of the counties we serve.
  • Good understanding of Medicare and Medicaid requirements (JACC, MLTSS, NJ Family Care)
  • 2 years of field experience preferred.
  • Experience with Microsoft Office (Word, Excel, PowerPoint) preferred
  • Experience with Google Sheets, Doc, Slides preferred.
  • Must possess the ability to quickly learn new IT systems and software.
  • Must be able to adjust and prioritize tasks.
  • Experience with an EMR strongly preferred
  • Experience analyzing, sorting and drawing conclusions from data strongly preferred

Location: Hunterdon Healthcare Partners
Schedule: Full-time, Day shift, 40 hours per week, Mon-Fri
