THE POSITION

DESCRIPTION OF WORK

You will provide on-going social work services to all assigned individuals at Warren State Hospital from admission to discharge. Services include, but are not limited to, processing admissions/discharges, completing initial assessments and/or re-assessments on all assigned individuals, providing written and verbal explanation of individuals’ rights on an annual basis, assisting with safe keeping of property, banking, and applying for benefits (MA, SS, SSI, SSDI). As a member of the Treatment Team you will participate in all ITP formulation, reviews, reformulations, and Community Support Plan meetings.

Work hours and additional information:

  • The Department of Human Services provides excellent benefits, pay increases, paid leave and holidays, as well as opportunities for career advancement.
  • Hours of work: 8:00am-4:00pm, Monday-Friday, 30-minute lunch
  • May change based on operational needs.
  • Travel: As needed

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

In compliance with Centers for Medicare & Medicaid Services (CMS) requirements, if you are selected for this position, you must show, at the time of hire, verification that you have received either a single-dose COVID-19 vaccination, or both doses of a two-dose COVID-19 vaccination, unless you are exempt from receiving a COVID-19 vaccination due to an ADA or religious accommodation.   • Minimum Experience and Training Requirements – A bachelor’s or master’s degree in social work or social welfare

Legal Requirements:

  • This position comes under the provisions of the Older Adult Protective Services Act. Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to PA State Police and FBI clearance.
