1. Demonstrates competency in providing clinical social worker services to inpatients, outpatients, family, staff and community members.
o Address impact of diagnosis on patient and family.
o Conducts patient and family counseling to enhance their ability to cope effectively.
o Ability to provide education/support to community groups and/or staff.
o Initiates advance care planning with patient and family.
o Able to provide day-to-day supervision for social work interns doing field placements.
o Able to provide assessment and referral for DCYF and BEAS.
o Able to provide assessment and referral for cases of domestic violence.
o Able to provide counseling/support to birth mothers in adoption cases.
o Responds to traumatic events and provides crisis intervention.
o Provides grief support/counseling as the situation dictates.
o Able to provide assessment and support to patients receiving palliative care.
o Ability to provide ongoing intervention and support to the patient/family unit in the outpatient setting.
2. Demonstrates competency in conducting psychosocial assessments and brief interventions.
o Obtains general history, including living situation; supports, psychological history, goals, strengths and coping skills.
o Uses assessment to develop and implement discharge plan.
o Ability to establish a plan of care for patients in the outpatient setting.
3. Demonstrates competency in effective communication.
o Liaison between family and healthcare team.
o Keeps necessary staff informed of high risk/problematic cases.
o Provides training and education to hospital staff.
o Facilitates completion of Advance Directives.
o Demonstrates the ability to communicate with persons from other cultural groups.
o Recognizes and respects diversity of co-workers and patients.
4. Demonstrates competency in the application of NASW principles.
o Practices in accordance with professional code of ethics.
o Seeks out Bioethics Committee, experts in field, and knowledgeable individuals for guidance in areas where ethical issues surface, conflict exists, or barriers prevent good care.
5. Demonstrates competency in discharge planning and outpatient referrals.
o Develops and advocates for discharge plan or the outpatient plan of care.
o Conducts patient, family and team conferences when needed.
o Addresses issues identified in treatment plan with family.
o Conducts efficient and timely discharge planning.
o Participates in daily rounds as appropriate.
o Provides information and referrals for Federal, State and community resources.
o Knowledgeable about Medicare regulations and related public funding sources, and local resources.
o Ability to collaborate and advocate with multiple community agencies, the hospital, and the outpatient team to create and work on a patient’s identifies plan of care.
6. Demonstrates competency in the performance of duties and responsibilities related to organizational skills, community resources, boundaries, and ongoing care of patients in the outpatient setting.
o Work is completed in a timely manner.
o Triages workload appropriately.
o Offers assistance to coworkers when able.
o Information sharing is done in a timely and organized manner.
o Ability to multitask prioritization of a caseload.

Qualifications


Experience Minimum Required

  • One year in healthcare setting

Experience Preferred/Desired

  • Two years experience including recent experience in acute health care setting

Education Minimum Required

  • Master degree in Social Work from an accredited college or university

Special Skills Minimum Required

  • Knowledge of medical terminology.
  • Ability to organize time and establish priorities; ability to communicate in written, electronic, and verbal formats; ability to give and receive information over the telephone; ability to make effective judgments and decisions based on objective criteria.
  • Ability to work in a stressful environment and handle sensitive issues; ability to maintain confidentiality; calm, reassuring, empathetic demeanor; flexibility, professional in interaction with people at all levels; ability to work with a wide range of patient conditions, personalities, and cultures; ability to provide good internal and external customer service.
  • Ability to operate a computer.

Special Skills Preferred/Desired

  • LCSW, membership in professional organization
