What You’ll Do

Summary:
The Social Worker will assist cancer patients from underserved populations in navigating the cancer care system at New Hanover Regional Medical Center and affiliated medical offices, thereby helping patients to close the gap between detection and appropriate treatment delivery. The Social Worker will assist cancer patients in receiving timely diagnoses, timely treatment, information and education relating to cancer treatment and clinical trials.

Responsibilities:
1. Responsible for explaining services. 2. Assess barriers to treatment. 3. Utilizes strong psychosocial skills to facilitate developmentally appropriate patient assessment, plan of care, interventions and evaluations processes. 4. Perform psychosocial assessments on new patients and as needed. 5. Provide counseling support and crisis intervention for patients and significant others, coping with difficult situations including intervention for suicidal ideations, abuse, neglect and domestic violence involving patients and families. 6 Arranges transportation and lodging. 7 Assists with insurance paperwork and addresses financial issues. 8. Assists with the commitment process. 9. Acts as Hospital liaison to the Department of Social Services for protective service cases. 10. Make appropriate referrals to Hospice, Home health and DME providers. 11. Facilitates/Assists patients/families with advanced directive planning to assure that “Advance Directive” and “Health Care Power of Attorney” forms have been filled out correctly and notarized. 12. Utilizes complex decision-making and solving skills that facilitate developmentally appropriate patient outcomes. 13. Assesses the patient’s financial needs and facilitates connecting them with community resources available while assisting with applications for assistance. 14. Arranges patient support groups, consulting with a mental health professional as needed. 15. Acts as an advocate for the patient to ensure their needs are met and rights protected. 16. Assists with establishing a home support system or alternative living placement plan, which is realistic in relation to the patients present and potential function, emotional, and developmental capabilities and is attainable in relation to the human, equipment, and community resources available (i.e. Homeless, lack of family support, etc.). 17. Assumes level of “expert” knowledge of community resources and facilitates connecting patient/families with needed services. Facilitates referrals for patients who have lack of financial resources to support their health care needs, connecting them with resources such as Department of Social Services for Medicaid, prescription support programs, and the like. 18. Participates in quality improvement activities 19. Participates in professional development activities. 20. Participate with interdisciplinary clinics 21. Facilitates completion of Level II PASARRs for patients seeking SNF placement per State guidelines

1. Education: Bachelors Degree in Social Work or related Human Services Field required. (Recommended Master’s Degree in Social Work or BSW with Master’s Degree in related human services field such as counseling of psychology). 2. Licensure / Certifications: None 3. Experience: Personal or professional experience with a diverse community. Minimum 2 years paid or volunteer work experience with an underserved population. Demonstrates standards of performance (ownership, teamwork, communication, compassion) that support patient satisfaction and principles of service excellence. Performs other duties as assigned.
